| 摘 要： | 目的:调查大肠埃希菌(Escherichia coli,ECO)尿液分离株中β-内酰胺酶和膜孔蛋白ompC基因的存在和变异情况。方法:收集南京医科大学第一附属医院2006年1月～2008年10月患者尿液标本中分离到的ECO菌共60株,采用聚合酶链反应(PCR)及序列分析的方法分析21种β-内酰胺酶基因和膜孔蛋白ompC基因。结果:在60株ECO菌中,TEM基因阳性51株(85.0%),经测序比对均为TEM-1-like;OXA-1群基因阳性6株(10.0%),经测序比对均为OXA-30-like;CTX-M-1群基因阳性38株(63.3%),经测序比对均为CTX-M-55-like。膜孔蛋白ompC基因经测序比对60株中有6株(10.0%)无突变,其余54株均有突变(突变率90.0%),其中6株经BLASTn比对确认为新的突变类型。结论:产CTX-M-55-like型β-内酰胺酶和膜孔蛋白ompC基因是本组ECO菌对β-酰胺类抗菌药物产生耐药的重要原因。6株ompC基因新的突变类型中,3株为同时携带TEM-1-like、CTX-M-55-like基因的新菌株;2株为携带了TEM-1-like基因的新菌株;1株...
