HPV-DNA检测在宫颈病变筛查中的意义

目的分析宫颈脱落上皮的HPV-DNA表达情况与TCT及宫颈组织病理活检之间的关系,旨在探讨出一种更为敏感与合理的宫颈病变筛查方案。方法 HPV-DNA的检测采用PCR体外扩增和DNA反向杂交相结合的DNA芯片技术,宫颈脱落细胞病变程度检测采用薄层细胞学检测系统（TCT）,宫颈组织病理活检采用经典方法;将HPV-DNA表达结果与TCT结果进行阳性率分析,采用统计学方法行×列χ2检验及χ2分割检验。结果炎症组HPV-DNA阳性率分别与ASC-US组及上皮内瘤变（SIL）组的HPV-DNA阳性率相比,炎症组低于后两组,差异有统计学意义（P〈0.05/3）;HPV联合TCT方案组的敏感性分别与HPV方案组及TCT方案组的敏感性相比,联合组的敏感性高于后两组,差异有统计学意义（P〈0.05/6）。结论宫颈癌前病变的HPV感染率高于宫颈炎症的感染率,HPV-DNA检测联合TCT检测是最有效的宫颈癌筛查手段。

Significance of Human Papillomavirus DNA Testing in Cervical Lesions Screening

Objective To approach a sensitive and rational screening proposal by analyzing the relations among the expression of HPV-DNA in cervical epithelia cells,the results of TCT and the cervical biopsy.Methods HPV-DNA testing was administered by DNA array technology combined PCR with reverse hybridization,cervical epithelia cells were detected by thinprep cytologic test(TCT),and cervical biopsy was performed by classic methods.The positive rates of HPV-DNA and TCT were analyzed statistically by Row×Column χ2 test.Results The positive rate of HPV-DNA in inflammation group was significantly lower than those in ASC-US group and SIL group,and the difference was statistically significant(P<0.05/3).The sensitivity in screening program of HPV testing associated with TCT was higher than that in the separate testing program,and the difference was statistically significant(P<0.05/6).Conclusions The screening program of HPV testing associated with TCT is the most effective screening method for cervical cancer.
