降钙素原检测在儿科感染中的临床意义

目的评价血清降钙素原（procalcitonin,PCT）在儿科感染中的实验诊断意义。方法对106例患儿按临床诊断把患儿分成细菌感染和非细菌感染2组进行白细胞（WBC）计数、C反应蛋白（CRP）、血清降钙素原（PCT）检测,PCT采用免疫色谱法测定,CRP采用免疫比浊法,WBC计数采用迈瑞BC5200血球计数仪测定。结果细菌感染组与非细菌感染组的PCT、CRP及WBC检测值均高于对照组,差异有统计学意义（P〈0.05）。细菌感染组的PCT及WBC检测值均高于非细菌感染组间,差异有统计学意义（P〈0.05）,而细菌感染组的CRP检测值与非细菌感染组比较差异无统计学意义（P〉0.05）。细菌感染组中PCT阳性率（49/59）明显高于非细菌感染组（6/47）,差异有统计学意义（P〈0.05）。结论 PCT检测能客观评估儿科感染的严重程度及治疗效果,对临床合理应用抗菌素有指导价值。

Clinical Significance of Procalcitonin Detection in Paediatric Infections

Objective To evaluate the clinical diagnostic value of procalcitonin(PCT) detection in paediatric infections.Methods According to the clinical diagnosis,106 child patients were divided into bacterial infection group(n=59) and nonbacterial infection group(n=47).PCT was detected by immunity chromatography.CRP was determined by turbidimetric immunoassay.And white blood cell was measured by blood cell counter(Mindray BC5200).Results The values of PCT,CRP and WBC detection in bacterial infection group and nonbacterial infection group were all higher than those in control group,and the difference was statistically significant(P<0.05).The values of PCT and WBC detection in bacterial infection group were both higher than those in nonbacterial infection group,and the difference was statistically significant(P<0.05).But no statistically significant difference was found in the value of CRP between bacterial infection group and nonbacterial infection group(P>0.05).The positive rate of PCT in bacterial infection group was significantly higher than that in nonbacterial infection group,and the difference was statistically significant(P<0.05).Conclusions PCT detection can objectively evaluate the severity and therapeutic effect of paediatric infections.It has a guiding value for rational use of antibiotics in clinical practice.
