二氧化碳气膀胱镜在成人血尿患者诊断中的临床应用价值

目的探讨二氧化碳(CO2)气膀胱镜在成人血尿患者诊断中的临床应用价值。方法以CO2为介质,对入选的215例成人血尿患者进行CO2气膀胱镜检查,其中男141例,女74例,肉眼血尿180例,镜下血尿35例。观察膀胱内壁、前列腺及尿道内等情况,必要时活检病理检查。结果所有患者均顺利完成检查或活检,手术时间13～24 min,术中无1例因出血而导致操作中断。患者术中术后均无明显CO2相关并发症。本组最终诊断为膀胱炎73例,膀胱癌46例,前列腺增生症27例,膀胱结石9例,上尿路出血17例,尿道炎10例,其中33例未见异常改变。结论 CO2气膀胱镜安全、可靠,操作视野清晰,对成人血尿患者尤其是严重血尿者的临床诊治有一定优势,值得临床推广应用。

Value of carbon dioxide for cystoscopy and its clinical application in the diagnosis of adult patients with hematuria

Objective To investigate the value of carbon dioxide for cystoscopy and its clinical application in the diagnosis of adult patients with hematuria.Methods The study group consisted of 215 adult patients with hematuria requiring CO2 cystoscopy.Among them,there were 141 male patients and 74 female patients.The number of patients with gross hematuria was 180,and patients with microscopic hematuria was 35.Results All examinations were successful with an average examination duration of 13-24 min.No examination was stopped as a result of bleeding.There were no CO2-related complications during preoperative and postoperative periods.Among 215 adult patients with hematuria,cystitis occurred in 73,bladder cancer in 46,benign prostate hyperplasia(BPH) in 27,bladder stones in 9,upper urinary tract hemorrhage in 17,urethritis in 10,and no abnormalities in 33.Conclusion Carbon dioxide for cystoscopy is a safe and effective technique that provides a good endoscopic view,and this technique would seem to have considerable advantages in the clinical diagnosis of adult patients with hematuria.
