抗乙肝免疫核糖核酸治疗慢乙肝的疗效观察

摘    要:目的 :探讨不同途径注射抗乙肝免疫核糖核酸 (iRNA)治疗慢乙型肝炎的效果。方法 :两组乙型肝炎病人分别用iRNA 4mg肌肉注射和从皮下注射于腋窝淋巴结周围 ,每日 1次 ,共 6 0次 ;对照组采用alb基因工程干扰素 (IFNα lb) 30 0万单位肌注 ,每日 1次 ,共 6 0次 ,治疗时间均为 6月。结果 :皮下淋巴结周围注射iRNA对乙肝病毒DNA(HBVDNA)、乙肝病毒e抗原 (HBeAg)阴转率分别达 40 91%、41 6 7% ,IFNα lb组为 44 44 %、47 6 2 % ;而肌肉注射iRNA为 10 5 3 %、13 0 4%。皮下淋巴结周围注射组疗效明显优于肌肉注射组 (P <0 0 5 ) ,其近期疗效与干扰素相近。结论 :皮下淋巴结周围注射iRNA治疗慢乙型肝炎效果明显 ,其近期疗效与干扰素相近 ,而价格较干扰素低 ,值得进一步观察评价

Abstract:Objective:To study the efficacy of administration of anti-HBV iRNA by different routes to treat chronic hepatitis B. Methods:Two groups of patients with HBV were iniected with anti-HBV iRNA (0.5 mg×60 times) in subcutaneous lymphnode and muscle.Control group was injected with IFN-α,300 IU×60 times.The treatment time was 6 months.Results:The rates of HBVDNA-negative and HBeAg-negative were 40.91% and 41.67% respectively by injecting anti-HBV iRNA in subcutaneous lympyhnode,those with INF-α were 44.44% and 47.62%,but those with intramuscular injection were 10.53% and 13.04%.The curative effect of anti-HBV iRNA by subcutaneous lymphnode injection was better than those by intramuscular injection (P<0.05),similar to that with INF-α.Conclusion:The effect of anti-HBV iRNA in the treatment of CHB by subcutaneous lymphnode injection showed comparatively good results.Its recently curative effect was similar to that of INF-α,but the price was lower than Interferon-α,the further observation was valuable.
