无创正压通气治疗稳定期COPD的临床疗效观察

摘    要:目的探讨无创正压通气(NPPV)治疗稳定期慢性阻塞性肺疾病(COPD)患者的临床疗效。方法选择急性发作入院治疗好转的COPD患者71例,随机分为无创呼吸机治疗组21例、家庭氧疗组24例、常规治疗组26例,分别治疗1、6个月后观察各组患者血气分析(Pa O2、Pa CO2)、肺通气功能(FEV1%、FEV1/FVC)等指标的变化。结果治疗6个月后,无创呼吸机治疗组Pa O2、Pa CO2、FEV1%明显改善,与治疗前比较差异均有统计学意义(P0.05)。治疗1、6个月后,家庭氧疗组Pa O2均有明显提高(P0.05),但PCO2和肺通气功能指标无明显改变;而常规治疗组动脉血气及肺功能治疗前后均无明显变化。结论在常规治疗基础上,无创正压通气可以改善COPD稳定期患者的血气及肺通气指标,其效果优于常规治疗和家庭氧疗组。

Abstract:Objective To investigate the clinical effects of noninvasive positive pressure ventilation on stable COPD patients.Methods Seventy one patients of COPD at stable stage were divided into three groups, which were NPPV group(n=21),domiciliary oxygen therapy group(n=24) and control group(n=26). One month and six months after treatment, the change of Pa O2, Pa CO2 and FEV1%, FEV1/ FVC was examined in the three groups. Results On the aspects of blood gas and lung ventilation index before and after treatment, there was no significance on the both in the control, a obvious change existed on the former while not on the latter in the domiciliary oxygen therapy and the index was improved on the both aspects in NPPV group. Conclusion On the basis of conventional treatment, noninvasive positive pressure ventilation can improved blood gas and lung ventilation index of patients with COPD stabilization, which was better than domiciliary oxygen therapy group.
