腹股沟疝补片修补术预防性使用抗菌药物的调查分析

目的:了解腹股沟疝补片修补手术围手术期抗菌药物的应用情况,为加强临床合理使用抗菌药物提供依据。方法:对2010年1月至12月271例腹股沟疝补片修补手术病例进行回顾性调查分析。结果:分析271例腹股沟疝补片修补手术病例围手术期使用抗菌药物情况,使用率为99.3%,共涉及10大类抗菌药物,使用频率最高的是二代头孢菌素(21.1%),其次是单酰胺环类(16.3%)。平均使用时间(4.1±1.3)(2～7)d,总用药时间超过48 h者213例。结论:存在围手术期抗菌药物使用不规范、用药时间长、选药档次偏高、用药时机不合理等现象,需进一步规范管理,促进抗菌药物的合理使用。

Preventive application of antibiotics in patients with inguinal hernia patch repair

Objective To study the present situation of application of antibiotics perioperatively in inguinal hernia repair with a patch device and to provide evidences for enhancing the rational use and standardized management of antibiotics.Methods Two hundred and seventy-one patients subjected to inguinal hernia repair using a patch device from Jan to Dec in 2010 were retrospectively investigated.Results The rate of antibiotics use during preoperative period was 99.3%.Ten classes of antibiotics were involved,among which the second-generation cephalosporins were the most frequently used with a rate of 21.1%,followed by aztreonam(16.3%).The duration of antibiotics use ranged from 2 to 7 days,the average time was(4.1±1.3) days.In 213 patients,antibiotics were used for more than 48 hours.Conclusions Perioperative prophylactic use of antibiotics tended to over-selection of high grade drug and the prolonged postoperative use of antibiotics.Therefore,it is necessary to intensify physicians′awareness on the rational use of antibiotics perioperatively.
