肥胖型高血压患者血清抵抗素的临床研究

目的探讨肥胖高血压病患者血清抵抗素水平与血脂、血压、体重指数、胰岛素敏感指数（ISI）等代谢紊乱及肿瘤坏死因子-α、高敏C反应蛋白、内皮素-1水平的关系。方法测定40例肥胖高血压患者及40例正常人血清抵抗素、肿瘤坏死因子-α、hs-CRP、ET-1人体测量变量值及生化指标。结果肥胖高血压组血清抵抗素、肿瘤坏死因子-α、高敏C反应蛋白、内皮素-1水平均显著高于对照组（P〈0.01）;血清抵抗素水平与BMI（r=0.63,P〈0.01）、WHR（r=-0.55,P〈0.01）、TG（r=-0.72,P〈0.01）、胰岛素（r=0.52,P〈0.01）、HOMA-IR（r=0.39,P〈0.01）、肿瘤坏死因子-α（r=0.41,P〈0.01）、高敏C反应蛋白（r=0.38,P〈0.01）、内皮素-1（r=0.36,P〈0.01）正相关,与HDL（r=-0.48,P〈0.01）负相关。结论肥胖高血压患者常伴有血清抵抗素增高与代谢紊乱;肥胖高血压及其代谢紊乱是多种因素相互作用的结果。

Clinical Research between Serum Resistin and Metabolic Disorder in Obesity-related Hypertension Patients

Objective To investigate the relationship between serum resistin and lipid,blood pressure,BMI,WHR,ISI,TNF-α、hs-CRP、ET-1 in obesity-related hypertension patients.Methods Totally 40 obesity-related hypertension and 40 age,sex-matched normal weight individuals were studied,serum resistin,TNF-α,hs-CRP,ET-1,serum lipid,serum glucose and insulin were tested and anthropometric parameters were measured.Results Obesity-related hypertension patients had higher serum resistin,TNF-α,hs-CRP,ET-1 than those in the normal weight group（P0.01）;The serum resistin was significantly positively correlated with BMI（r=0.63,P0.01）、WHR（r=-0.55,P0.01）、TG（r=-0.72,P0.01）、INS（r=0.52,P0.01）、HOMA-IR（r=0.39,P0.01）、TNF-α（r=0.41,P0.01）、hs-CRP（r=0.38,P0.01）、ET-1（r=0.36,P0.01）,but negatively correlated with HDL-c（r=-0.46,P0.01）.Conclusion Obesity related hypertension patients often had higher serum resistin and metabolic disorders.Obesity related hypertension and metabolic disorders is the result of the interaction of various factors.
