乳腺癌术后小剂量芬太尼自控静脉镇痛的临床应用

目的探讨小剂量芬太尼用于乳腺癌术后患者自控静脉镇痛（PCIA）的效果和安全性。方法 60例乳腺癌术后患者随机分为常规剂量芬太尼组（N组）和小剂量芬太尼组（S组）,分别使用常规剂量芬太尼和小剂量芬太尼进行PCIA,记录术后2h、1d、2d各时点VAS、镇静评分、并发症和患者的镇痛满意度等。结果两组各时点VAS评分差异无统计学意义。镇静评分、恶心呕吐评分在术后2h、1d两组间差异有统计学意义,S组优于N组。镇痛满意度评分在术后2h两组间差异有统计学意义,S组优于N组。结论小剂量芬太尼用于乳腺癌术后PCIA安全有效。

Application of Small Dose Fentanyl PCIA after Breast Cancer Surgery

Objective To investigate the efficacy and safety of small dose of Fentanyl for PCIA after breast cancer surgery.Methods Totally 60 cases of breast cancer patients after surgery were randomly divided into normal dosage Nepal Fentanyl group（N group） and small dosage Fentanyl group（S group）,separately with conventional dose and small dose of Fentanyl for PCIA,recording VAS,sedation score,nausea and vomiting score,complications and patient’s satisfaction of analgesia in 2 hours,1 day and 2 days.Results The difference of VAS score at each time point between the two groups has no statistical significance.Sedation score and nausea and vomiting score in the postoperative 2 hours and 1 day has statistical difference between the two groups,which indicates that S group is better than N group.Satisfactions of analgesia in the postoperative 2 hours were statistically different between the two groups,which prove that S group is better than N group.Conclusion Small dose of Fentanyl for PCIA after breast cancer surgery was safe and effective.
