摘    要:目的 探讨CAPN-10基因Indel-19与腹膜透析患者代谢综合征(MS)危险因素的关系.方法 用PCR-RFLP(聚合酶链反应-限制性片断长度多态性)法检测30例健康者(A组)、30例稳定的腹膜透析患者(B组)CAPA-10基因Indel-19多态性,生化技术测定FBG(空腹血糖)、TG(三酰甘油),HDL-C(高密度脂蛋白胆固醇)及FINS(空腹胰岛素)水平.用HOMA-IR法评价胰岛素抵抗,同时对每个受试对象进行MS危险因素评估.结果 B组患者收缩压(SBP)、舒张压(DBP)、FBG、FINS、TG及HOMA-IR水平较A组高(P<0.05).A、B两组比较CAPN-10基因Indel-19基因型分布无明显差异(P=0.580).B组基因型为2/2的患者较基因型为1/1、1/2患者有较多MS的危险因素.结论 腹膜透析患者多同时存在胰岛素抵抗、脂质代谢紊乱及高血压.CAPN-10基因Indel-19可能与腹膜透析患者血糖、血脂及高血压有关.其可能机制是影响腹膜透析患者血糖、脂质代谢,加重患者胰岛素抵抗的程度,但其具体作用机制尚有待于进一步研究.

Abstract:【Objective】 To explore the relationship between Indel-19 polymorphisms of CAPN-10 gene and peritoneal dialysis patients.【Methods】 PCR-RFLP(polymerase chain reaction-restrictive fragment length polymorphism) was used to test the genotype of Indel-19 of CAPN-10 gene.The people being tested include 30 normal people and 30 stable continuous ambulatory peritoneal dialysis(CAPD) patients.FBG,FINS,TG and HDL-C were also tested.【Results】 The frequencies of different genotype were same in 2 groups(P =0.580).SBP,FINS,quantity and score of risk factors of MS are higher in patients with genotype 2/2.HOMA-IR and quantity of risk factors of MS maybe were the main effects.【Conclusion】 Indel-19 of Calpain-10 gene maybe one of the genetic factors contributing to the occurrence of end-stage renal disease.The disorder of glucose,lipid metabolism and insulin resistance maybe are possible causes,but the other possible theories need to be further studied.
