癌痛患者抑郁情绪的调查及影响因素分析

目的:研究癌痛患者抑郁情绪的发生率并进一步分析影响抑郁情绪的因素。方法:调查对象为2009年12月至2010年12月在我院疼痛康复科住院的癌症患者。本研究入组患者234例,均经病理学、细胞学以及影像学证实确诊为癌症患者。其中伴有癌性疼痛的(癌痛组)124例,平均年龄60.90±12.20岁,不伴有疼痛的(对照组)110例,平均年龄60.95±12.09岁。全部被试者完成一般资料问卷、数字疼痛强度量表(numerical rating scale,NRS)、中国癌症患者生活质量调查问卷(qualityof life que-stionnaire,QLQ)、抑郁自评量表(self-rating depression scale,SDS)。结果:癌痛组抑郁情绪的发生率为70.97%,明显高于对照组(36.36%,P<0.01);伴有中、重度疼痛的癌症患者抑郁分均值高于伴有轻度疼痛的抑郁分均值(P<0.05);生活质量与癌痛患者抑郁程度呈负相关(r=-0.760,P=0.000);多元逐步回归分析发现,生活质量、疼痛程度、癌症病程入选回归方程,回归方程为:Y(抑郁程度)=5.761-1.200 X13(生活质量)-0.242 X10(疼痛程度)-0.125 X7(癌症病程)。结论:疼痛和生活质量是影响癌痛患者抑郁情绪的重要因素。积极有效地控制疼痛、提高生活质量,对于改善癌痛患者的心理状态有重要意义。

A SURVEY AND ANALYSIS OF DEPRESSION PATIENTS WITH CANCER PAIN

Objective: To investigate the prevalence of depression of patients with cancer pain and to further analyze the factors influencing depression.Methods: Cancer patients were investigated in our hospital.234 cases were selected,including 124 paients with cancer pain(cancer pain group) and 110 patients without pain(control group).All these cases were cancer patients confirmed by pathology,cytology and imaging.All the cases themselves completed the following questionnaires: general condition questionnaire,numerical rating scale(NRS),quality of life questionnaire(QLQ) for Chinese cancer patients and selfrating depression scale(SDS).Results: The prevalence of depression of cancer pain group was 70.97%,which was significantly higher than that of control group(36.36%,P < 0.01).The mean score of depression of cancer patients with either middle pain or severe pain was higher than that of cancer patients with mild pain(P < 0.05).Quality of life was negatively correlated with the degree of depression(r =-0.760,P = 0.000).Multiple stepwise regression analysis showed quality of life,pain intensity and duration of cancer had been selected.The regression equation was: Y(depression) = 5.761-1.200 X13(quality of life)-0.242 X10(pain intensity)-0.125 X7(duration of cancer).Conclusion: Pain and quality of life are important factors for depression of patients with cancer pain.It is of great significance to control pain actively and enhance quality of life for improving the mental state of patients with cancer pain.
