慢性肝炎肝组织内血管病变与病理学的关系

摘    要:研究慢性肝炎肝组织中血管病变与肝组织病理学的关系。方法:1047例慢性乙型肝炎肝活检标本,连续切片分别作HE及网状、胶原及弹力纤维组化染色,光镜观察肝组织病理学分级与血管炎症、增生、破坏及阻塞的关系,结果:1.血管炎症及增生都与肝组织病理学分级呈正相关(P<0.05);但在G_3与C_4组无显著性差异(P>0.05)。2.血管破坏及阻塞也随G_1~C_4逐级加重(P<0.05);但G_1和C_2组无重度破坏及重度阻塞者,只有G_3和G_4才有重度破坏阻塞者较多(P<0.05)。结论:1.慢性肝炎肝组织内血管炎症、增生与肝组织病理学炎症相伴发生,呈正相关,可能有互为因果关系;2.血管的重度破坏和重度阻塞可能是导致肝组织病理学桥形坏死或大块坏死的重要原因之一。

Abstract:To explore the relations between hepatic vascular lesions and histopathology in chronic hepatitis.Methods: Liver biopsy specimens of 1047 cases with chronic hepatitis B were examined by light microscope.The specimens section were stained with routine technique and histcx'hemical techniques for reticulin,collagen and elastic fibers.Attention was focused on the relations of histopathology gradings to the inflammation, proliferation, damage and occlusion of the blood vessels.Results: 1.The inflammation and proliferation of the blood vessels in the liver were both positively related to the hepatic histopathologic gradings(P<0.05).2.So did the damage and occlusion of blood vessels to the grad-ings( I'< 0.05).Conclusions: 1.The inflammation and proliferation of blood vessels in the liver tissues did occur in chronic viral hepatitis.2.Severe damage and severe occlusion of the bloud vessels may be the main causes of bridging necrosis and massive necrosis in the hepatic histopathology.
