医用创面封闭胶在左肝外叶切除术中的止血作用

目的研究医用创面封闭胶在左肝外叶切除术中的止血作用。方法将2003年6月￣2005年6月连续施行左肝外叶切除的患者随机分为二组,A组中肝断面用传统的缝扎技术止血,B组中用医用创面封闭胶止血;记录术中的止血时间、失血量、手术时间和术后胆漏、出血情况。结果B组中彻底止血的时间(20±7)min较A组明显缩短([53±13)min,P<0.01],B组中的失血量(252±98)mL也明显少于A组([437±110)mL,P<0.01],A组中有2例患者于术后出现胆漏,而B组中无胆漏,二组的手术时间无差异性[A组(:205±47)分,B组(:[196±53)min;P<0.25]。结论医用创面封闭胶能在左肝外叶切除的手术中达到迅速而持久的止血作用。

Application of medical obturation glue as effective hemostatic agent in left lateral hepatectomy

[Objective] To study medical obturation glue as a hemostatic agent in left lateral hepatectomy.[Method] Between June 2003 to June 2005, 80 consecutive patients with hepatolithiasis were underwent left lateral hepatectomy. The patients were randomly devided into two groups and their cut parenchymal surface were treated with conventional suture techniques (Group A; n=40) or Medical obturation glue (Group B; n=40) to achieve hemostasis. The hemostatic time, blood loss, biliary leakage, postoperative bleeding and length of surgery were recorded. [Result] The time until complete hemostasis was achieved was shorter for Group B [(20±7) min] versus Group A [(53±13) min; P ＜0.01]. The amount of hemorrhage was less in Group B (252±98 mL) versus Group A [(437±110) mL; P ＜0.01]. 2 patients in Group A presented a postoperative biliary leakage, however, no postoperative biliary leakage was observed in Group B. And no postoperative bleeding was observed in two groups. There was no difference between the two groups in the length of surgery [Group A: (205±47) min vs Group B: (196±53) min; P =0.25]. [Conclusion] The medical obturation glue provides immediate and durable hemostasis in in left lateral hepatectomy.
