100例老年肺结核合并肺部真菌感染临床分析

目的探讨老年肺结核合并肺部真菌感染的流行情况、危险因素、临床特点、诊断治疗。方法对2006—2010年收治的100例老年肺结核合并肺部真菌感染患者进行回顾性分析。结果老年肺结核合并肺部真菌感染患者多为病程较长、病变范围广泛、用过广谱抗生素、抵抗力下降的复治者,在临床上常被原发病掩盖,随着肺内病变的増多真菌感染的发生率増加,确诊有赖于病原学检查,以白色念珠菌为主。结论老年肺结核合并肺部真菌感染患病人数呈上升趋势,必须重视2病并存的的防治。

Analysis on the clinical characteristics of 100 cases of elderly pulmonary tuberculosis complicated by fungal infection

Objective To explore the epidemic condition,risk factors,clinical characteristics,diagnosis and treatment in elderly patients with pulmonary tuberculosis complicatied by fungal infection. Methods 100 paitents with elderly pulmonary tuberculosis complicated by fungal infection hospitalized from 2006 to 2010 in Shandong Chest Hospital were analyzed retrospectively. Results Most of the cases were retreated patients who had a long course of disease, extensive lesions, broad-spectrum antibiotic used and immunosuppressed status. The symptom and sign of fungal infection were covered by primary disease in clinical. The incidence of fungal infection increased with the lung lesion progression and the candida albicans was the common pathogen. Confirmed diagnosis was depended on etiological detection. Conclusion The number of the elderly patients with pulmonary tuberculosis complicated by fungal infection is increasing, we should pay more attention to the prevention and treatment of the coexisted disease.
