Ⅱ型糖尿病患者白内障超声乳化手术的临床观察

目的 :观察Ⅱ型糖尿病患者 (NIDDM)白内障超声乳化白内障摘除 (PHACO)的优点。方法 :选取解放军总医院近 2年以来行白内障手术的患者 ,将其中行PHACO手术的Ⅱ型糖尿病患者 32例 35眼 (A组 ) ,与同期行白内障囊外摘除 (ExtracapsularCataractExtraction ,ECCE)的Ⅱ型糖尿病患者2 8例 30眼 (B组 )和行PHACO的非糖尿病的白内障患者 2 4 4例 2 5 3眼 (C组 )的术后反应和术后视力进行比较分析。结果 :①术后反应 :瞳孔区纤维素样渗出和色素沉着 ,A组 6眼 ,B组 13眼 ,C组有 2 4眼 ,A、B两组之间差异有显著性意义 ,A、C两组差异无显著性意义。②术后视力 :术后反应消退后 ,所有患者全部脱残 ,视力≥ 0 .5者A组有 2 7眼 ,B组有19眼 ,C组有 2 16眼 ,A组与其他两组间差异无显著性意义。结论 :超声乳化白内障摘除手术 (PHACO)对Ⅱ型糖尿病患者具有优越性 ,但应该注意患者的选择和加强术后随访

Investigation of cataract patients with NIDDM after PHACO

Objective:To investigate the merits of phacoemulsification(PHACO) on cataract patients with NIDDM.Methods:304 cases(318 eyes) of patients with senile cataract who had undergone surgery during the previous 2 years at the General Hospital of PLA were chosen,32 cases(35 eyes) of NIDDM treated were with PHACO and 28 cases(30 eyes) were treated with extracapsular cataract extraction(ECCE) and 244 cases(253 eyes) without DM were treated with PHACO. Postoperative inflammation and eyesight were compared and analyzed.Results:Fibroid exudation and pigmentation in the pupil were found in 6 eyes in the PHACO group with NIDDM and 13 eyes in the ECCE group with NIDDM. There was a statistical difference between the two groups. The conditions were less evident in the PHACO group than in the ECCE group. There were 24 eyes with fibroid exudation and pigmentation in the PHACO group without DM,but there was no statistical difference when compared to the PHACO group with NIDDM. Eyesight for all eyes exceeded the standard for visual disability after postoperative inflammation vanished. There were 27 eyes whose eyesight exceeded 0.5 in the PHACO group with NIDDM,19 in the ECCE group with NIDDM and 216 in the PHACO group without DM. However,there was no statistical difference between the PHACO group with NIDDM and the other two groups.Conclusion:The value of PHACO can be seen in senile cataract patients with NIDDM in our study,but the selection of patients and postoperative observation should be monitored more closely.
