正畸种植体支抗的临床应用及其在正畸过程中稳定性研究

摘    要:目的:探讨正畸种植体支抗的临床应用情况及正畸后种植体稳定情况。方法:将我院收治的60例错颌畸形患者,随机分为对照组和观察组,对照组采用常规治疗后佩戴支抗正畸治疗,观察组在常规治疗的基础上采用微种植体植入正畸治疗。分析两组治疗效果及患者中切牙倾角差、磨牙移位、上中切牙凸距差,正畸种植体稳定性等。结果:观察组显效率、总有效率显著高于对照组,差异有统计学意义(P〈O.05)。观察组患者上中切牙倾角差、上中切牙凸距差显著高于对照组,差异有统计学意义(P〈O.05)。观察组磨牙位移显著低于对照组,差异有统计学意义(P〈O.05)。观察组出现3例微种植体松动,经取出后2个月重新植入,全部稳定未见再脱落者。微种植体脱落原因为:口腔卫生状况不佳,引导钻预备孔直径太大,种植体偏离方向。所有患者种植及复种植后均经头影扫描重叠图检查提示上前牙显著直立及内收,软组织侧面形显著提高,均迭强支抗效果。结论:微型种植体正畸治疗,疗效优于常规治疗,治疗后上中切牙倾角差、上中切牙凸距差显著高于对照组,观察组磨牙位移显著低于对照组,术后种植体稳定性良好,使用方便,值得临床推广应用。

Abstract:Objective : To study the clinical application of the orthodontic implant anchorage and stabili- ty conditions after orthodontic implant. Method: 60 cases of wrong micromaxillary deformity patients, were randomly divided into control group and observation group, a resistance of orthodontic treatment to be used in the control group after normal treatment, micro planting implants to be used in the observation group after normal treatment. Analysed two groups of treatment effect and patients incisive Angle difference, grind one" s teeth shift, in incisive convex distance difference, orthodontic implant stability, etc. Result: The show ef- ficiency, the total effective rate group in significantly higher than those in the control group, the difference was statistically significant ( P 〈 0.05 ). The observation group patients in incisive Angle difference, in inci- sive convex distance difference significantly higher than those in the control group, the difference was statisti- cally significant ( P 〈 0.05 ). The observation odontoprisis displacement significantly lower than those of the control group, the difference was statistically significant ( P 〈 0.05 ). The observation group appeared 3 ca- ses of micro implant loosening, the remove 2 months to implant, all stability did not see those off again. Mi- cro implant deciduous causes for: oral hygiene condition not good, guide drill prepared hole diameter is too big, implant deviation direction. All patients after planting and complex planting are subject to head shadow scanning superposed graph examination of the forward tooth significant upright and adduetion, the soft tissue profile form has improved markedly, reached a strong resistance to effect. Conclusion: Miniature implant or- thodontic treatment, the curative effect is better than the conventional treatment, after treatment in incisive Angle difference, in incisive convex distance difference significantly higher than those in the control group, the observation group grind one's teeth displacement significantly lower than those of the control group, the implant stability is good, easy to use, worth to application at clinical.
