氨氯地平贝那普利片(Ⅱ)降低家庭自测血压的跟踪随访研究

摘    要:目的:评价单片复方制剂氨氯地平贝那普利片(Ⅱ)治疗原发性高血压患者的降压效果和对靶器官的保护作用。方法:在山东大学齐鲁医院招募50例原发性高血压患者。患者洗脱2周后,口服氨氯地平贝那普利片(Ⅱ)1片,每天1次,治疗6个月。患者分别于每天清晨7点和晚上8点测量坐位血压,观察治疗前后的血压、空腹血液和尿液相关化验指标及超声心动图中心脏结构和功能的变化。结果:治疗6个月后,患者的清晨及夜间的家庭自测血压明显下降,数据差异有统计学意义(P<0.01);治疗后的尿蛋白肌酐比值水平显著下降,差异有统计学意义(P<0.05);治疗后的左室舒张早期血流峰值速度(E)与二尖瓣环根部舒张早期峰值速度(e')比值(E/e')、左室质量指数、室间隔厚度、左室后壁厚度、左房收缩末期内径、左心室舒张末期内径的水平均明显降低,差异有统计学意义(P<0.01);治疗6个月后的血糖水平较治疗前明显降低,差异有统计学意义(P<0.01)。所有患者在随访期间未出现严重不良反应。结论:氨氯地平贝那普利片(Ⅱ)不仅降压疗效显著,而且对心脏、肾脏有保护作用,值得临床推广。

Abstract:OBJECTIVE To evaluate the antihypertensive effect and target organ protection of amlodipine besylate and benazepril hydrochloride tablets (Ⅱ) in patients with essential hypertension.METHODS A total of 55 patients with essential hypertension in Qilu Hospital of Shandong University were recruited. However, five patients were lost to follow-up and 50 patients were eventually included. After a two-week of washout period, all patients were given amlodipine besylate and benazepril hydrochloride tablets (Ⅱ) once a day for 6 months. The patients measured their sitting blood pressure at 7 a.m. and 8 p.m. by themselves. Laboratory indexes related to fasting blood and urine, as well as the structural and functional changes of heart by echocardiography were monitored before and after treatment.RESULTS After 6-month treatment, sitting blood pressure in the morning and at night decreased significantly compared with that before treatment (P<0.01), urinary albumin-creatinine ratio decreased significantly(P<0.05), the levels of E/e', left ventricular mass index, interventricular septum, left ventricular posterior wall thickness, left atrium end-systolic diameter and left ventricular end-diastolic diameter were significantly lower than those before treatment (P<0.01), and blood glucose levels decreased significantly as well (P<0.01). No serious adverse reactions occurred in 50 patients with essential hypertension during the treatment.CONCLUSION Amlodipine besylate and benazepril hydrochloride tablets (Ⅱ) not only has significant antihypertensive effect, but also play a protective role in heart and kidney, which is worthy of clinical promotion.
