上矢状窦旁脑膜瘤切除和上矢状窦重建15例

目的：探讨上矢状窦旁脑膜瘤的手术治疗方法。方法：对15例上矢状窦旁脑膜瘤患者采用显微手术方法切除，同时行受累上矢状窦壁切除＋重建上矢状窦，其中，10例患者直接缝合重建受损矢状窦，5例患者硬脑膜或骨筋膜修补重建受损矢状窦。结果：15例患者手术结果满意，术后恢复良好，无死亡病例；偏瘫定位症状的患者，术后病情好转，预后良好；所有患者随访5个月～6年，1例患者术后2年脑膜瘤复发，行二次手术治疗，其余14例均未见复发，复发率为6．7％。结论：采用显微外科技术行上矢状窦旁脑膜瘤镜下全切除，同时行受损矢状窦重建，可有效治疗上矢状窦旁脑膜瘤

15 CASES OF PARASACITTAL MENINGIOMA RESECTION AND RECONSTRUCTION OF SUPERIOR SAGITTAL SINUS

Objective To investigate the surgical treatment of parasagittal meningioma. Methods 15 cases of parasagittal meningioma patients were operated with microsurgical methods, in the mean time the affected superior sagittal sinus wall resection and reconstruction of superior sagittal sinus was done. Among them direct suture and reconstruction of the affected superior sagittal sinus were done in 10 patients, compensation and re- construction of the affected superior sagittal sinus with dura mater or bone fascia in 5 cases. Results Operation results were satisfying in all the 15 cases, with ideal post-operation recovery and no death cases; Improved condition and ideal prognosis in patients with hemiplegia location symptoms; All the patients underwent follow-up 5 months to 6 years, among whom 1 case the meningioma reoccurred 2 years after operation and second time operation was done, and other 14 cases never reoccurred with reoccurance rate 6.7%. Conclusion Endoscopic complete resection of parasagittal meningioma with microsurgical methods, in the mean time reconstruction of affected superior sinus was done, can cure parasagittal meningioma effectively.
