甲状腺手术体位综合干预的观察与研究

目的：观察甲状腺手术中应用钢丝罗纹气管插管以降低颈部过伸的角度并进行综合体位干预，以减轻或避免病人术后颈过伸脑循环紊乱综合征的效果。方法将410例甲状腺手术病人随机分成两组，对照组216例，采用普通气管插管和常规体位；观察组194例，术中应用钢丝罗纹管并行综合体位干预。观察两组病人术后颈过伸脑循环紊乱综合征的发生率。结果观察组头痛、恶心呕吐、腰背痛等脑循环紊乱综合征的发生率均明显低于对照组（P＜0．01）。结论甲状腺手术中应用综合干预的手术体位，可避免或降低头痛、腰背痛等脑循环紊乱综合征的发生。

Observation and study for thyroid surgery postural comprehensive intervention

Objective To observe the application of steel rib for thyroid surgery and to reduce neck hyperexten-sion position angle and conduct comprehensive intervention ,and to reduce or avoid postoperative neck hyperexten-sion and cerebral circulation disorder syndrome .Method 410 patients with thyroid surgery was set and divided into two groups ,216 cases in control group ,ordinary endotracheal intubation and conventional postural was set for inter-vention .194 cases in observation group ,Intra -operative integrated steel ribbed tube and comprehensive postural intervention .Incidence of postoperative neck hyperextension cerebral circulation disorder syndrome were observed for both group of patients .Result Incidence rate of headache ,nausea ,vomiting ,back pain and other disorders of cerebral circulation syndrome in observation group was significantly lower than that of the control group （ P〈0 .01） .Conclusion The occurrence of headache ,back pain and other disorders of cerebral circulation syndrome can be reduced or avoided by applying comprehensive intervention surgical position during thyroid surgery .
