应用三丙烯微球行豚鼠子宫动脉栓塞术的实验研究

摘    要:【目的】探讨应用三丙烯微球(Embosphere)行正常豚鼠子宫动脉栓塞术的可行性【方法】25只成年雌性豚鼠随机分为两组?子宫动脉铸型组10只,用于行盆腔血管造影及血管铸型术,了解雌性豚鼠子宫动脉起源,行程,长度,直径及分支等解剖学特征;子宫动脉栓塞组(UAE组)15只,用于行双侧子宫动脉微球栓塞的技术研究及病理分析【结果】双侧子宫动脉主干由髂内动脉发出,于同侧宫角旁的结缔组织中迂曲上行并发出8-10支弓状支,主干和弓状支的血管直径分别为(0.32 ± 0.027) mm及(0.14 ± 0.01) mm?UAE组15只豚鼠成功行双侧子宫动脉栓塞术,术中(40 ~ 120) ,m及(100 ~ 300) .m Embosphere的用量分别为(0.033 ± 0.003) mL和(0.015 ± 0.002) mL?病理切片可见子宫动脉弓状支,浆膜下二级分支及部分三级分支动脉内均有栓塞微球存在,子宫肌层及内膜无缺血坏死病理改变【结论】应用三丙烯微球栓塞豚鼠子宫动脉具有可行性,栓塞效果良好,可应用于豚鼠子宫肌瘤模型UAE的基础研究

Abstract:Abstract: 【Objective】To explore the feasibility of applying trisacryl gelatin microspheres(Embosphere) to uterine arterial embolization (UAE) on normal guinea pig. 【Methods】 25 female adult guinea Pigs were divided into two groups: one group was 10 guinea pigs for uterine artery casting mould, which were used to make pelvic angiography and vascular casting mould, and to demonstrate the anatomy characteristics of uterine arteries’ source、running shape、length、diameter and branches of female adult guinea pig;another group was 15 guinea Pigs for UAE, which were used to practise technical research and make pathological analysis by bilateral uterine arterial embolization with microspheres. 【Results】The trunks of uterine arteries were erupted by internal iliac arteries, ranning up circuitously in the connective tissue beside the cornu uteri, and erupted 8 to 10 arcuate branches. The trunk and arcuate branches’ diameter were 0.32±0.027mm and 0.14±0.01mm respectively. The 15 guinea pigs of the UAE group were succeeded in operating UAE. The dosage of 40-120µm and 100-300µm Embosphere were 0.033±0.003mL and 0.015±0.002mL respectively during the operation. On the pathological section, the microspheres could be found in the uterine arteries’ arcuate branches、second branches within the subserosa and third branches, but the myometria and endometria had no abnormal pathological change. 【Conclusion】It is feasible to use trisacryl gelatin microspheres to operate uterine arterial embolization for guinea pig and the embolization effects are satisfactory. So the embolization technique could be use in basic study of UAE for guinea pig uterine fibroid model.
