功能性肠病患者的心理评价

目的研究心理因素在功能性肠病(FBDs)发病中的地位和作用。方法应用Zung焦虑自评量表(SAS)和抑郁自评量表(SDS)对肠易激综合征(IBS)、功能性腹泻(FDi)和功能性便秘(FC)患者和正常对照者进行评分,分析各组之间的差异。结果FBDs患者中单纯具有焦虑倾向者占18.64%,单纯具有抑郁倾向者占21.82%,同时具有焦虑及抑郁倾向者占23.18%;FBDs患者SAS和SDS得分显著高于对照组;FBDs患者SAS量表得分女性显著高于男性;D-IBS、A-IBS、FC组SAS得分均显著高于对照组;D-IBS、C-IBS、A-IBS、FC组SDS得分均显著高于对照组。结论功能性肠病患者多伴有焦虑、抑郁倾向;女性患者更容易伴有焦虑倾向;C-IBS患者较FBDs其他亚型患者多伴有抑郁倾向;FDi组患者较少伴有心理障碍。

Evaluation of psychological factors on patients with functional bowel disorders

Objective To evaluate the psychological effect on patients with functional bowel disorders(FBDs).Methods Psychological characteristics were measured with Zung's self-rating anxiety scale(SAS) and self-rating depression scale(SDS).Results The patients of FBDs with simple anxiety accounted for 18.64%,the patients with simple depression accounted for 21.82%,the patients with anxiety and depression accounted for 23.18%,the scores of SAS and SDS in FBDs patients were significantly higher than that in the control;the scores of SAS in female patients were significantly higher than that in the male patients;the scores of SAS in constipation predominant irritable bowel syndrome(C-IBS),functional constipation(FC),alternating diarrhea and constipation irritable bowel syndrome(A-IBS) groups were significantly higher;the scores of SDS in diarrhea predominant irritable bowel syndrome(D-IBS),C-IBS,A-IBS,FC were significantly higher than that in normal controls.Conclusion The psychologic obstacle such as anxiety and depression associate with FBDs;the female patients with FBDs carries increased ratio with anxiety;the ratio with depression in C-IBS is higher than that in other subtypes with FBDs;the patients with functional diarrhea(FDi)carry lower ratio with psychologic obstacles.
