放疗联合卡铂增敏治疗鼻咽癌疗效分析

目的探讨卡铂在鼻咽癌放射治疗中的增敏作用。方法 89例鼻咽癌患者随机分为两组。单纯放疗组 (对照组 )4 3例 ,卡铂联合放疗组 (治疗组 ) 4 6例 ,两组放疗方式及剂量相同。治疗组在放疗第 1、3、5周放疗前应用卡铂 ,每次10 0mg ,静滴 ,每周 5次。 结果鼻咽肿瘤及颈淋巴结转移灶完全消退率治疗组明显高于对照组 (P <0 .0 5 )。 3年内局部复发及远处转移治疗组明显低于对照组 ,分别是 10 .9%、15 .2 %和 2 7.9%、34.9%,均有显著性差异。 3年生存率治疗组、对照组分别是 78.3%、5 8.1%(P <0 .0 5 )。结论放疗联合适量卡铂化疗 ,起到一定的放射增敏作用 ,并能抑制或延缓肿瘤复发及远处转移 ,可提高鼻咽癌患者的生存期。

Clinical analysis of radiotherapy combined with carboplatin for sensitization of nasopharyngeal carcinoma

Objective To evaluate the radiosensitizing effect of carboplatin in treating patients with nasopharyngeal carcinoma. Methods 89 patients of nasopharyngeal carcinoma were randomly divided into simple radiotherapy group of 43 patients(control group) and radiotherapy combined with carboplatin group of 46 patients(treatment group).The radiotherapy method and dosage was the same in the two groups. Carboplatin was given to treatment group before radiotherapy in 1st,3rd and 5th week,100mg/f,iv. Results The complete response(CR) rates in the nasopharynxe and the cervical metastasises were higher in treatment group than in control group(P<0.05). The 3 year local relapse rate was 10.9% in treatment group and 27.9% in control group(P<0.05),respectively. The 3 year distant metastasis rate was 15.2% in treatment group and 34.9% in control group(P<0.05),respectively. The 3 year survival rate was 78.3% in treatment group and 58.1% in control group(P<0.05),respectively. Conclusion Radiotherapy combined with carboplatin can increase radiosensitivity and inhibit or delay relapse and distant metastasis so to improve the survival period of nasopharyngeal carcinoma.
