无创心功能监测对肺心病诊断和疗效判断的意义

摘    要:目的 研究无创心功能监测对慢性肺源性心脏病诊断和治疗效果判断的临床意义。方法 对肺心病患者急性期、缓解期及健康人刘照组分别进行肺动脉压力和右心功能监测,所得参数经统计学处理。结果 肺心病急性期右心室收缩压(RVPS)、肺动脉收缩压(PAPS)、肺血管总阻抗(SVR)、肺小动脉阻抗(PVR)、右心排指数(RCI)、右室等容指数(RCRI)均显著高于缓解期水平,差异有显著意义(P〈0.01)。肺心病缓解期SVR、PVR显著高于正常对照组.差异有显著意义(P〈0.01),RCRI增高,差异有显著意义(P〈0.05),而RVPS、PAPS、RCI间无明显差异(P〉0.05)。结论 无创心功能监测可作为诊断肺心病及判断疗效的一种敏感方法。

Abstract:Objective To investigate the clinical significance of noninvasive heart function monitor of diagnosis and therapy of chronic pulmonary heart disease.Methods The PAP and right heart function of exacerbation period and stable period of chronic pulmonary heart disease,and that of normal healthy controls were monitored.T test were used for statistics.Results Compared exacerbation and stable period of chronic pulmonary heart disease with stable period,the RVPS,PAPS,SVR,PVR,RCI,RCRI,the difference was significant(P<0.01).Compared stable period of and normal healthy controls,the SVR,PVR had significant difference(P<0.01),PCRI also had significant difference(P<0.05),but RVPS,PAPS,RCI hadn't have significant difference(P>0.05).Conclusion Noninvasive heart function monitor is a sensitive way to diagnose and to judge therapy effect of chronic pulmonary heart disease.
