高龄股骨粗隆间粉碎性骨折治疗体会

目的：探讨高龄粗隆间粉碎性骨折患者的有效治疗方法。方法：回顾性分析60例股骨粗隆间粉碎性骨折的治疗,分为髓内固定治疗组和骨水泥型人工股骨头置换组,比较两组手术治疗效果并进行统计学分析。结果：骨水泥型人工股骨头置换组在手术时间、出血量、卧床时间、负重时间及骨折愈合时间上均少于Gamma钉内固定组（P〈0.05）,差异有统计学意义;置换组的术后并发症发生率明显下降,术后关节恢复优良率明显提高（P〈0.05）。结论：人工股骨头置换治疗股骨粗隆间粉碎性、不稳定性骨折操作简单、手术时间短、出血少、功能恢复快、可早期负重,显著降低了并发症,疗效确切。

Treatment of Intertrochanteric Fracture of Femur in Elderly

Objective：To investigate the effective treatment of intertrochanteric fracture of femur in elderly patients. Meth- ods：Retrospective analyzed the treatment of intertrochanteric fracture of femur in elderly patients （n= 60）. The patients were divided into the intramedullary fixation group and femoral head prosthetic replacement group by bone cement. Results： Compared with the intramedullary fixation group, the operating time, volume of bleeding, bed time, weight-bearing time, fracture healing time and the incidence of complication were less in the femoral head prosthetic replacement group（P〈0. 05）, and the joints recovery after surgery was better （P〈0.05）. Conclusion：The treatment of intertrochanteric fracture of femur in elderly patients by femoral head prosthetic replacement is easy to operate and effective, reducing the operating time, volume of bleeding, the time of recovery and the incidence of complication.
