COPD合并肺间质纤维化的特点及其临床意义

目的 探讨COPD合并肺间质纤维化的特点及临床意义。方法 对近5年来本院收治的经临床确诊的COPD合并肺间质纤维化患者36例的病史、临床表现、胸部体征、胸片、CT片及部分高分辨CT、肺功能及血气检查结果进行分析。结果 COPD合并肺间质纤维化的临床表现介于两种疾病之间，肺功能检查为混合性通气功能障碍或先为阻塞性通气功能障碍，之后逐渐演变成混合性通气功能障碍。胸片及CT则兼有两种疾病的特点。结论 COPD合并肺间质纤雏化临床的确存在，且使原来单一的疾病表现不典型而有其独特的临床表现。肺功能、胸片、CT、尤其是高分辨CT对COPD合并肺纤维化的诊断有重要价值。

The clinical characteristics and clinical significance of chronic obstructive pulmonary disease with pulmonary fibrosis

Object To explore the clinical characteristics and clinical significance of chronic obstructive pulmonary disease(COPD)with pulmonary fibrosis. Methods 36 patients with COPD and pulmonary fibrosis were analyzed by patients history, clinical symptoms, chest feature, radiographic features, computed tomography(CT) of chest , high resolution computed tomography(HRCT), lung function test and arterial blood gas analysis.Results Clinical characteristics of COPD with pulmonary fibrosis were between COPD and pulmonary fibrosis.Lung function test showed mixed ventilation disturbances or mixed ventilation disturbance appeared behind obstruction ventilation disturbance. Characteristic of COPD with pulmonary fibrosis of radiography and CT was between COPD and pulmonary fibrosis.Conclusions COPD with pulmonary fibrosis has unique clinical manifestation,Lung function test, CT and HRCT are valuable tools to diagnose COPD with pulmonary fibrosis.
