急诊静脉溶栓治疗急性脑梗死临床观察

目的：了解急诊溶栓治疗急性脑梗死患者的院内延迟时间及不同治疗时间的治疗效果。探讨缩短病人院内延迟时间的方法及溶栓治疗的有效时间窗，方法：将80例急性脑梗死患者随机分为两组，每组40例，急诊溶栓组经CT或MRI确诊后立即应用尿激酶静脉溶栓治疗，住院溶栓组则在住院后进行。结果：急诊溶栓组病人来诊至溶栓治疗开始的平均时间为58min，发病6h内接受溶栓治疗者19例（47．5％），住院溶栓组来诊至溶栓治疗开始的平均时间为120min，发病6h内接受溶栓治疗者12例（30％），两组治疗前后神经功能缺损积分比较有显著性差异，组间比较亦有显著性差异（P＜0．05），发病6－48h内溶栓仍有效，结论：急诊尿激酶静脉溶栓治疗急性脑梗死可缩短院内延迟时间，减轻神经功能缺损程度。

THE CLINICAL STUDY OF EMERGENT INTRAVENOUS THROMBOLYSIS FOR TREATMENT OF ACUTE CEREBRAL INFARCTION

Objective:To evaluate the efficacy of emergent intravenous thrombolysis for treatment of acute cerebal infarction in different time and to investigate effective time window of thrombolysis.Methods:80 patients were randomly divided into two groups.Fourty patients in emergent thrombolysis group were treated immediately with urokinase after given CT or MRI examination.The other 40 patients were given a similar management after hospitalized.Results:The emergent thrombolysis group took average 58 minutes from seeing doctor to accepting treatment.Nineteen patients were treated with thrombolysis with in 6 hours after cerebal infarction (47 5%).The hospitalization group took average 120 minutes from seeing doctor to accepting treatment.Twelve patients were treated with thrombolysis within 6 hours after the appearance of the disease(30%). There is a significant difference between the two groups in nerve function damnification and so is between pre and post treatment in each group( P <0 05)?Conclusion:Emergent intravenous thrombolysis for treating acute cerebral infarction may shorten delated time in hospital and reduce the severity of nerve function damnification.
