人工髋关节置换术后患肢早期疼痛原因分析

[目的]探讨人工髋关节置换术后患肢早期疼痛的原因及防治。[方法]分析178例人工髋关节置换术病例,其中全髋关节置换56例(71髋),人工股骨头置换122例。分析早期疼痛的原因、临床特点及防治措施。[结果]15例出现患肢早期疼痛,其中切口感染1例,切口血肿1例,下肢深静脉栓塞5例,神经损伤3例,原发膝关节病变引起的疼痛3例,原有创伤1例,异位骨化1例。[结论]严格手术适应证,完善术前准备,正确手术操作,缩短手术时间,术后正确处理,早期积极功能锻炼是减少患肢早期疼痛发生的关键。

ANALYSIS OF EARLY PAIN AFTER ARTIFICIAL REPLACEMENT OF HIP JOINT

[Objective]To study the cause of early pain after artificial replacement of hipjoint,andits protection and prevention.[ Methods]178 cases of artificial replacement of hipjoint,including 56 cases of total hipreplacement and 122 cases of femoral headre-placement,were studied respectively about their causes,clinical features,and preventive measures.[Results]Early pain were foundin15 cases,including 1 case of infection at superficial part,1 case of incisional hematoma,5 cases of lowli mb phlebothrombosis,3 casesof nerve injury,3 cases of pri mary kneejoint pain,1 case of injury,and 1 case of heterotopic ossification.[Conclusion]The keytore-duce the early painis to abide by the operationindication strictly,prepare for and performthe operation perfectly,shortenthe opera-tion ti me,give the proper care after operation,and encourage the patients to do the dirigationin ti me.
