桡骨上1／3骨折并桡骨小头脱位：附3例报告

报告了8例桡骨上1/3骨折并桡骨小头脱位。认为是前臂创伤中一种新类型骨折,其损伤机制和临床变位情况,既不同于孟氏骨折的任何一型,也不同于桡骨干中1/3、下1/3骨折和前臂双骨折,而是由一种向上并旋前的传导暴力所造成。由于骨折处恰在旋后肌附着点上,旋后肌的突然收缩致折端向前成角,及桡骨头后脱位,并形成向后移位的撕脱性折块。治疗上首选闭合整复,采用近折端对远折端的手法,术后小夹板固定前臂于中立位,可取得满意效果。

Upper-1/3 radial fracture with radial head dislocation: A report of 3 cases

Three cases of upper-1/3 radial fracture with radial head dislocation are reported. The authors consider that this is a new-type fracture in forearm in juries, The mechanism and clinical metachoresis are different both from those of any type of Monteggia's fracture and from upper 1/3, or middle-1/3, or lower-radial fracture and forearm double fracture. The injury is caused by an upward prone conducting violence. Since the fracture site is just located in the place of the attachment of the supine muscle, the sudden contraction of the muscle causes a foreward angulation displacement of the fragment and a posterior dislocation of the radial head, and forms an avulsed and backward displaced fragment. In treatment, closed manipulation is a firstly selected therapy, and the proximal fragment-to-distal-fragment maneuver should be ap- plied. A satisfactory result can be obtained by splint fixation of the forearm in a neutral position.
