胃肠恶性肿瘤及其切缘组织中p53基因突变和p53蛋白表达

目的 探讨胃肠道恶性肿瘤及其切缘组织中p53基因突变和p53蛋白表达与手术切缘的安全性关系。方法 用PCR－SSCP和免疫组化S－P方法检测20例胃癌和10例大肠癌标本和切缘组织中p53基因突变和p53蛋白表达情况。结果 （1）胃肠道恶性肿瘤组织中有很高的p53基因突变率（63％）和p53蛋白阳性表达率（53．33％）。（2）肿瘤组织中p53蛋白表达是切缘组织中p53蛋白表达的前提。（3）p53基因突变和p53蛋白表达随肿瘤切缘距离的增加而呈现极显著的递减趋势。结论 就p53基因突变和p53蛋白表达而言，距离肿瘤边缘5cm可定为肿瘤的分子切缘。

Clinical study on p53 gene mutation and p53 protein expression in gastrointestinal carcinomas and peripheral tissues

Objective To detect the p53 gene mutation and p53 protein expression in the tumor center and peripheral tissues of patients with gastrointestinal cancers, and explore the security of tumor surgical margins. Methods Detected the expression of p53 gene mutation and p53 protein expression in tumor tissues,the approximal and distal peripheral tissues from the tumor margin of 20 gastric and 10 colorectal carcinomas with PCR-SSCP and immunohistochemistry S-P methods respectively. Results (1) The mutation rates of p53 gene and the expression of p53 protein in tumor tissues are high. (2) Only the p53 protein expressed in tumor tissue could it be expressed in tumor margin tissue. (3) The lower and lower of the p53 gene mutation and the p53 protein expression with the increased way from tumor margin. Conclusion Considered p53 gene mutation and p53 protein expression, 5 cm can be defined as molecular margin.
