内镜下经腋窝径路乳房假体置换术的初步探讨

目的探讨内镜辅助下经腋窝径路乳房假体取除与重置的方法及可行性。方法硅凝胶假体隆乳术后2例,聚丙烯酰胺水凝胶注射隆乳术后1例,符合乳房假体取除及重置的适应证,予腔镜下经腋窝切口游离进入胸大肌前(后)间隙,分离包膜取除假体材料,并重新植入硅凝胶乳房假体。结果手术均获成功。手术时间70￣200min,出血量均少于30mL,住院时间3d。无皮下或纵隔内气肿、血肿或感染及空气栓塞等并发症,随访18个月以上无假体移位或包膜挛缩。结论在熟练掌握腔镜微创外科技术的前提下,内镜下乳房假体置换术是安全可行的。

Research of endoscopic assisted replacement augmentation mammoplasty through axillary incision

Objective] To explore the flexibility of endoscopic assisted augmentation mammoplasty replacement through axillary incision. Methods] Three patients who had to remove the implant after augmentation mammoplasty were underwent the operation, which including removal of the capsular and replacement of the implant transaxillary incision with laparoscopic technique exclusively. Results] All of the operations were performed successfully. The operating time was 1.17 to 3.33 hours, the blood lose less than 30 mL, and no intraoperative or postoperative complications were observed. The patients were discharged within three days later after operation. An 1.5 to 3 years follow-up shows no capsular contracture and removal after the procedure. Conclusion] With more experiment and improvement of the surgical technique, the endoscopic assisted implant replacement through axillary incision after augmentation mammoplasty is secure and acceptable .
