彩色多普勒诊断乳腺良、恶性肿块的价值

摘    要:目的 分析彩色多普勒在诊断乳腺良恶性肿块中的价值。方法 恶性肿块 6 3例 (浸润性导管癌 5 9例 ,单纯癌 2例 ,导管内癌 2例 ) ,良性肿块 46例 (纤维腺瘤 43例 ,导管内乳头状瘤 1例 ,小叶增生 2例 )。全部病例均经手术及病理证实。结果 恶性肿块无血液 ( 0级 )检出率 4.8%,显著低于良性肿块 ( 5 2 .2 %)。Ⅱ级以上血流检出率 ,恶性肿块为 80 .9%,显著高于良性肿块 ( 10 .9%)。恶性肿块阻力指数为 0 .74± 0 .0 5 ,良性肿块阻力指数为 0 .6 4± 0 .0 7,恶性肿块搏动指数为 1.6 8± 0 .49,良性肿块搏动指数为 1.0 0± 0 .0 7,两者之间均有明显差异 (P <0 .0 1)。恶性肿块最大血流速度高于良性肿块 ,但差异不大 (P >0 .0 5 )。彩色多普勒诊断乳腺良恶性肿块的灵敏性、特异性、准确率分别为 95 .2 %、89.1%、92 .3%。结论 彩色多普勒对乳腺良恶性肿块的诊断有一定的价值。

Abstract:Objective To analyse the value of color Doppler in diagnosis of benign and malignant breast masses.Method The malignant breast masses involved 63 patients.Fifty-nine of them were infiltrative ductal carcinoma,two were simple carcinoma,and two were intraductal carcinoma.There were also 46 benign masses cases.Among them,43 were fibroadenoma,one was intraductal papilloma,2 were lobular hyperplasia.All cases were confirmed by surgery and pathology.Results The finding rate of non-blood stream of malignant tumors(zero degree) was 4.8%.It was evidently less than that in the benign masses(52.2%).The blood flow finding rate above second degree masses in the malignant masses(80.9%) was higher than that in the benign masses(10.9%).The resistance index(RI) of malignant tumors was 0.74±0.05,the benign masses was 0.64±0.07.The pulsation index(PI) of malignant tumors was 1.68±0.49,the benign masses was 1.00±0.07.The significant difference was found in both indexes(P<0.01).There was few difference between benign and malignant masses for peak velocity(P>0.05).The sensitivity for diagnosis of color Doppler was 95.2%,with a specificity of 89.1% and an overall accuracy of 92.3%.Conclusions Color Doppler plays an important role in the diagnosis of benign and malignant breast masses.
