青少年正畸治疗中牙龈蛋白R的变化对牙龈炎性反应的影响

摘    要:目的 分析青少年在正畸治疗中牙龈蛋白R(gingipain R,Rgp)变化对牙龈炎症的影响.方法 选择青少年牙龈健康者60名,分别取矫治器戴入前与矫治器戴入后第2个月的龈沟液,应用16S rDNA PCR技术检测各样本中的牙龈卟啉单胞菌及Rgp,同时记录牙龈指数,用x2检验比较牙龈卟啉单胞菌、Rgp各组间检出率,分析牙龈卟啉单胞菌、Rgp的检出率与牙龈指数之间相关性.结果 矫治器戴入前与戴入后第2个月的牙龈卟啉单胞菌的检出率分别为31.67%和56.67%,两者之间有极显著性差异(P<0.01);Rgp检出率分别为15.79%和44.12%,两者之间具有显著性差异(P<0.05).矫治器戴入后牙龈炎症指数Ⅰ级与戴入前牙龈炎症指数0级检出率之间有显著性差异(P<0.05).结论 青少年在正畸治疗中,矫治器的戴入使得牙龈卟啉单胞菌及Rgp检出率明显增加,易出现牙龈炎性反应,提示口腔卫生宣教的重要性.

Detection of gingipain R and the relationship with gingivitis index in the subgingival of teenagers

Abstract:Objective To explore the correlation between the gingipain R (Rgp)and inflammatory reaction in the process of orthodontic treatment.Methods Totally 60 orthodontic healthy teenagers were randomly chosen from the orthodontic.Before orthodontic treatment,after treatment,and subgingival plaques were collected simultaneously at each time point.16S rDNA PCR technique was used to detect P.g and Rgp.Using SPSS 17.0 software for statistical analysis,P.g and Rgp detection rate comparison between each group with x2 test.The relationship between the detection rate and gingival inflammation with Spearman rank correlation analysis.Results The detection rate of P.g before orthodontic treatment was 31.67 percent and after treatment was 56.67 percent.They have significant difference (P〈 0.05).The detection rate of Rgp before orthodontic treatment was 15.79 percent and after treatment was 44.12 percent.Positive correlation (P〈0.05) was observed between before orthodontic treatment and after treatment.The gingivitis index 0 before orthodontic treatment and after treatment the gingivitis index Ⅰ have statistically significant (P 〈 0.05).Conclusions Fixed orthodontic appliances caused Porphyromonas gingivalis and Gingipain R increased and lead gingival inflammatory reaction.
