封开地区城乡儿童幽门螺杆菌感染的现状及相关因素流行病学调查

目的了解封开地区城乡两地儿童幽门螺杆菌(Hp)感染率,并探讨Hp感染的相关因素。方法对封开地区城乡631名无症状儿童进行问卷调查,并进行血清Hp抗体和粪便Hp抗原检测。结果本地区儿童平均Hp感染率为55.9%,其中城区儿童Hp感染率为45.8%,农村儿童Hp感染率为66.5%。经单因素2分析后显示城区和农村Hp感染率呈现明显差异。并进行多因素条件Logistic回归分析表明年龄、与大人共用刷牙口杯、人均居住面积、照顾者有胃病史及共同进餐者有胃病史是城乡两地儿童共同的高危因素。结论本地区儿童Hp感染仍处于一个较高水平,且城乡差距较大,并随着年龄有递增的趋势,且呈现家庭内聚集现象,这与个人卫生生活习惯、家庭社会经济状况存在密切相关。

EPIDEMIOLOGICAL INVESTIGATION OF HELICOBACTER PYLORI AND ITS CORRELATIVE FACTORS IN CHILDREN LIVED IN FENGKAI AREA

Objective To investigate infection rate of Helicobacter pylori（Hp） and its correlative factors in children lived in urban and rural area of FengKai. Methods 631 asymptomatic children were surveyed by question- naires, also serum of Hp antibody and Hp stool antigens （HpSA） were taken. Results The average infection rate of Hp was 55.9% in Fengkai area,in which the urban area was 45.8% and the rural area was 66.5%. Univariate analy- sis X2 was showed significant difference between urban and rural. Multiple Longistic regression analysis indicated the age,toothcup - sharing with other family members at home, living space, stomach trouble of adults who looked after the child, eating with the adults who had a history of gastritis or peptic ulcer were the relevant factors for Hp infection. Conclusion The region of Hp infection in children remains a high level, and Hp infection clusters within families. Personal hygiene practices and family socio - economic status were also involved in the transmission of Hp to children.
