聚合人脐带血血红蛋白(PolyPHb)对兔心肌缺血/再灌注损伤的保护研究

摘    要:目的探讨聚合人脐带血血红蛋白(PolyPHb)对新西兰大耳白兔心肌缺血/再灌注损伤的保护效果。方法采用随机分组对照研究的方法,将30只雄性新西兰大耳白兔随机平均分为假手术组、对照组和实验组;对照组和实验组分别注射5 ml生理盐水和PolyPHb溶液(8 gHb/dl)做预处理,通过临时阻断冠脉前降支实现心肌缺血;观察缺血30 min、再灌注90 min后的心脏功能和肌钙蛋白Ⅰ释放水平。结果实验组与对照组比较,兔左室收缩压:(82.59±8.13)mmHg vs(61.39±7.92)mmHg(P0.05);左室内压最大上升速率:(3.76±0.73)mmHg/msvs(2.23±0.89)mmHg/ms(P0.05);左室内压最大下降速率:(-3.08±0.49)mmHg/ms vs(-1.89±0.65)mm-Hg/ms,P0.05):左室舒张末压(9.35±1.82)mmHg vs(18.21±2.43)mmHg(P0.01);肌钙蛋白Ⅰ释放水平:(8.71±1.90)ng/ml vs(17.83±1.72)ng/ml(P0.05)。结论 PolyPHb预处理对兔心肌缺血/再灌注损伤具有明显保护作用。

Abstract:Objective To investigate whether polymerized human placenta hemoglobin(PolyPHb) given before ischemia protects in vivo rabbit heart against ischemia/reperfusion(I/R) injury.Methods Thirty male New Zealand rabbits were randomly divided(n=10 per group) to sham group,control group(pretreatment with 5 ml saline) or PolyPHb group(pretreatment with 5 ml 8 gHb/dl PolyPHb).Rabbit hearts were subjected to 30 minutes ischemia by occlusion of left anterior descending,followed by 90 minutes reperfusion.The cardiac function and cardiac troponin-I(cTnI) release were measured.Results As compared to the control group,the left ventricular systolic pressure(LVSP),left ventricular maximum LVDP increase(+dp/dt) and decrease rate(-dp/dt) of the PolyPHb group were greatly improved [(82.59±8.13)mmHg vs(61.39±7.92)mmHg,P<0.05;(3.76±0.73)mmHg/ms vs(2.23±0.89) mmHg/ms,P<0.05;(-3.08±0.49)mmHg/ms vs(-1.89±0.65)mmHg/ms,P<0.05],and the left ventricular end-diastolic pressure(LVEDP) was significantly reduced when compared to the control group[(9.35±1.82)mHg vs(18.21±2.43)mmHg,P<0.05].In addition,the cTnI release was also markedly inhibited in the PolyPHb group as compared with the control group [(11.71±1.90) ng/ml vs(17.83±1.72)ng/ml,P<0.05].Conclusion PolyPHb pretreatment provides a profound cardioprotective effect on in vivo rabbit heart.
