鼻中隔黏膜下剥离划痕术治疗常年性变应性鼻炎

目的探讨鼻中隔黏膜下剥离划痕术治疗常年性变应性鼻炎的疗效。方法局麻鼻内窥镜下，对伴有鼻中隔偏曲者行鼻中隔黏膜下矫正术，伴有鼻甲息肉病变者去除部分或大部分鼻甲；对不伴有鼻中隔偏曲者，按筛前神经鼻内侧分布区，行鼻中隔黏膜下剥离划痕术，去除大部下鼻甲前端黏膜。术后常规口服抗生素治疗7d，局部滴减充血剂，鼻腔生理盐水冲洗。结果50例患者近期有效率达98％，无效率2％。结论鼻中隔黏膜下剥离划痕术能阻断筛前神经末梢，减轻血管张力，降低腺体分泌，去除了部分鼻甲组织增加鼻腔通气面积，同时破坏部分鼻腔黏膜而减弱了变态反应的强度。从而有效的控制了鼻痒、流涕、连续喷嚏等急性症状和以持续鼻塞为主的慢性症状，提高了患者的生活质量。

Methods Downwards nose endoscope using the local anaesthesia, applying to the operation on the person with the nasal septurn deviation under the mucous membrane orthotics,and the partner who has nose armor polypus lesion dislodge the part or most of nose armor; to the person without nasal septum deviation, using the operation that is peeled off the scratch skill under the mucous membrane across the nose to dislodge the most of mucous membrane nose armor fore-end according to the distribution area before sieving nerve nose inside. Take an antibiotic orally curing 7 days after the operation, droplet decongestant partly and wash the cavum nasi with the normal saline. Results Among 50 patients the efficiency amounting to 95% , inefficiency only 5% in the near future. Conclucion Peeling off the scratch skill under the mucous membrane being able to hinder cutting off sieve front nerve-end in the nose, will relieve the blood vessel tension, reduce gland secretion, get rid of the part nose armor organizes to increase breathable area in cavum nasi and destroy the part of mucous membrane in cavum nasi but having weakened an allergy at the same time. There will be an effective method to control impatient symptoms such as itching , shedding the mucus , the continuous sneeze and chronic systems primarily with the stuffy nose continuing, so it improve the patient＇ s life.
