改良美容切口联合胸锁乳突肌瓣在腮腺良性肿瘤切除术中的应用

目的:探究改良美容切口联合胸锁乳突肌瓣在腮腺肿瘤切除术中的应用。方法:选取2016年8月~2020年7月在我院接受腮腺良性肿瘤切除术治疗的104例腮腺良性肿瘤患者,按照随机分组法分为改良美容切口组与联合治疗组,各52例。改良美容切口组选择改良美容切口,联合治疗组选择改良美容切口并采用胸锁乳突肌瓣进行组织缺损区修复。记录患者术后引流量、切口愈合时间,评估患者面神经功能H-B面神经功能分级量表以及BISS评分,以及术后并发症及满意度。结果:与改良美容切口组相比,联合治疗组术后引流量较高,但两组比较无统计学差异(P>0.05)。与改良美容切口组相比,联合治疗组切口愈合时间较低(P<0.05)。与改良美容切口组相比,术后联合治疗组H-B面部神经功能值高,联合治疗组BISS评分低(P<0.05)。联合治疗组并发症总发生率低于改良美容切口组,满意度显著高于改良美容切口组(P<0.05)。结论:在腮腺良性肿瘤切除术中选择改良美容切口联合胸锁乳突肌瓣,切口愈合时间较短,术后能够改善患者面神经功能,提升身体状态,患者满意程度高。

Application of modified wrinkle removal and modified cosmetic incision combined with sternocleidomastoid muscle flap in parotid benign tumor resection

Objective:To explore the application of modified cosmetic incision combined with sternocleidomastoid muscle flap in parotid tumor resection.Methods:A total of 104 patients with benign parotid tumors who underwent parotid resection in our hospital from August 2016 to July 2020 were randomly divided into modified cosmetic incision group and combined treatment group,with 52 patients in each group.In the modified cosmetic incision group,the modified cosmetic incision was selected and in the combined treatment group,the modified cosmetic incision was selected in the parotid benign tumor resection and the sternocleidomastoid muscle flap was used to repair the tissue defect area of the patients.The drainage volume and wound healing time were recorded and compared.The facial nerve function of patients was evaluated by H-B facial nerve function grading scale.The BISS score was used to evaluate all patients before and after operation.Results:Postoperative drainage was higher in the combined treatment group compared with the modified cosmetic incision group, although there was no statistical difference between the two groups(P>0.05). Compared with the modified cosmetic incision group, the incision healing time in the combined treatment group was lower(P<0.05).Postoperative H-B facial nerve function value was higher in the combined treatment group than in the modified cosmetic incision group,BISS score in the combined treatment group was lower than(P<0.05).The total incidence was lower than that of the modified cosmetic incision group,and the satisfaction of the combined treatment group was significantly higher(P<0.05).Conclusion:Modified cosmetic incision combined with sternocleidomastoid muscle flap was selected for the treatment of parotid benign tumors during parotid benign tumor resection.The healing time of the incision was shorter,and the facial nerve function of patients could be improved,their physical status could be improved,and their satisfaction level was high.
