闭式冲洗留置负压引流治疗口腔颌面部间隙感染患者的疗效观察

Curative effect of closed washing indwelling vacuum sealing drainage on patients with oral and maxillofacial space infection

Objective:To study the clinical value of closed washing indwelling vacuum sealing drainage(VSD)in the treatment of oral and maxillofacial space infection.Methods:The clinical data of 89 patients with oral and maxillofacial space infection from June 2016 to September 2019 were retrospectively collected.The patients were divided into routine group(conventional incision and drainage,n=42)and closed washing indwelling VSD group(n=47).The curative effects of the two groups were evaluated,changes in white blood cell count(WBC),neutrophil count(NEUT)and high-sensitivity C-reactive protein(hs-CRP)levels before and after operation were compared between the two groups.Changes in the range of abscess before and after operation were measured.The degree of postoperative restriction of mouth opening was evaluated,and the Wong-Baker facial expression scale was used to evaluate the wound pain degree at different time after operation.Results:The grade of curative effect of VSD group was better than that of routine group(P<0.05).WBC,NEUT and hs-CRP in VSD group were lower than those in routine group at 3 days after operation(P<0.05).One week after operation,the abscess diameter of VSD group was smaller than that of routine group,while the mouth opening degree and mouth opening relief value were higher than those of the routine group(P<0.05).Pain scores of both groups at 24 h,72 h and 1 week after operation were lower than those before operation(P<0.05),and pain scores of VSD group were lower than those of the routine group at different time after operation(P<0.05).Conclusion:The overall efficacy of VSD in the treatment of oral and maxillofacial space infection is better than traditional incision and drainage,which can quickly reduce the patient’s systemic inflammatory response,promote postoperative wound healing,relieve restriction of mouth opening,reduce the frequency of dressing changes and the patient’s wound pain.
