Tei指数评价左室舒张功能的临床研究

目的探讨心肌运动指数（Tei指数）评价左室舒张功能的临床价值。方法依据二尖瓣口血流频谱比值E／A〈1及二尖瓣环间隔、侧壁2个位点的舒张早期、晚期组织多普勒平均运动速度比值Ea／Aa〈1，选取左室舒张功能减低患者58例（病例组），同时选取56例心脏结构及功能正常者（对照组），应用脉冲多普勒二尖瓣口、主动脉瓣口血流频谱及组织多普勒二尖瓣环运动频谱测定计算两组研究对象的Tei指数。结果病例组和对照组的脉冲多普勒Tei指数[（0．56±0．11）vs（0．41±0．08）]、间隔TDI Tei指数[（0．57±0．11）VS（0．45±0．08）]、侧壁TDITei指数【（0．55±0．11）VS（0．41±0．08）】、平均TDITei指数[（0．56±0．10）vs（0．43±0．07）】相比较，其差异均具有统计学意义（P〈0．05）。结论Tei指数能够简单、敏感、准确评价左室舒张功能，可为临床提供更可靠的诊断信息。

Clinical research on assesment of left ventricular diastolic function with Tei index.

Objective To investigate the value of Myocardial Performance Index （Tei index） for estimating left ventricular diastolic function. Methods Fifty-eight patients with diastolic dysfunction were selected as the study group according to the ratio of mitral valve early （E） and late （A） diastolic flow velocity E/A〈1, and the ratio of 2 mitral annular sites （lateral and septal） everage early （Ea） and late （Aa） diastolic motion velocity Ea/Aa〈1 by tissue Doppler imaging. In the same period, 56 healthy adults were selected as the control group. Tei index was calculated severally from pulse Doppler flow of mitral valve and aortic valve and PW-TDI recording of the septal and lateral comer of the mitral annular. Results Tei index was higher in the study group than the control group： pulse Doppler Tei index [（0.56±0.11） vs （0.41±0.08）], septal PW-TDI Tei index [（0.57±0.11） vs （0.45±0.08）], lateral PW-TDI Tei index [（0.55± 0.11） vs （0.41±0.08）, average PW-TDI Tei index [（0.56±0.10） vs （0.43±0.07）], all with statistically significant difference （P〈0.01）. Conclusion Tei index is valuable and simple for estimating left ventricular diastolic function with high sensitivity and could provide more reliable clinical diagnostic information.
