电刺激对端侧吻合后面神经再生作用的实验研究

摘    要:目的 研究神经电刺激(electrical nerve stimulation,ENS)对面神经周围支端侧吻合后促神经再生的作用。方法 24只成年家兔随机分为3组,双侧面神经颊支切断后与同侧个外膜开窗的颈支作端侧吻合。右侧为实验组,术后给予ENS,共5周。左侧不给电刺激,作为对照组,分别于术后3,5,15周取材,进行大体观察和面神经功能评分,神经组织学,电生理、透射电镜等检查。结果 各组实验侧颊支有髓鞘神经数,运动神经传导速度,面神经功能评分均高于对照侧(P<0.05)。实验侧颊支神经成熟程度成于对照侧。结论 ENS在提高神经端侧吻合后侧支萌出率和减轻失神经肌肉萎缩方面有积极的作用。

Abstract:Objective To study the effect of electrical nerve stimulation(ENS) on peripheral facial nerve regeneration after end to side neurorrhaphy. Methods 24 rabbits were randomly divided into three groups. The cheek branch of facial nerve on both sides was transected and then its distal end sutured to the cervical nerve branch with epineurial window in an end to side fashion. ENS was given to the right side postoperatively for 5 weeks. The left side serve as control without electrical stimulation. Gross appearance observation and the score of facial nerve function, histological, electrophysioligical, and transmission electron microscopic examination were carried out at 3,5 and 15weeks postoperatively. Results Compared with the control side, the expermental side had more regenerating myelinated nerve fibers, faster motor nerve conduction velocity, higher score of facial nerve function. The maturity of myelin sheath of the cheek branch was also superior. Conclusion ENS had beneficial influence on promoting peripheral facial nerve collateral sprouting after end to side neurorrhaphy and reduce denervation muscle atrophy.
