| 摘 要： | 目的定量检测乙型肝炎患者血清中乙型肝炎病毒（HBV）共价闭合环状DNA（cccDNA）、HBV DNA、乙型肝炎表面抗原（HBsAg）和乙型肝炎e抗原（HBeAg）并分析其相互关系,探讨定量检测血清中HBV cccDNA的临床应用价值。方法随机选取HBV感染者83例,采用实时荧光定量聚合酶链反应（FQ-PCR）检测血清中HBVcccDNA及HBV DNA,同时采用化学发光法定量检测血清中HBsAg和HBeAg。结果 83例患者中HBV DNA阳性62例,其中HBV DNA载量≥105拷贝/mL者40例,血清HBV cccDNA阳性28例（70.00%）;HBV DNA载量〈105拷贝/mL者22例,血清HBV cccDNA阳性2例（9.09%）,不同DNA载量组HBV cccDNA差异有统计学意义（P〈0.01）。HBeAg阳性组和阴性组HBV cccDNA阳性率分别为70.00%（28/40）和4.65%（2/43）,两组差异有统计学意义（P〈0.01）。HBV cccDNA定量值与HBV DNA、HBsAg呈正相关（P〈0.01）。结论血清中HBV cccDNA水平可同时反应血清HBV DNA和HBsAg水平,血清HBV cccDNA含量可作为观察HBV复制情况和评价抗病毒疗效的相关血清指标。

| Abstract: | Objective To analyze the correlation between Hepatitis B virus（HBV） covalently closed circular DNA（cccDNA）,HBV DNA,hepatitis B surface antigen（HBsAg）and hepatitis B e antigen（HBeAg） in serum of hepatitis B patients,and explore the clinical application value of HBV cccDNA quantitative detection.Methods 83 cases of HBV-infected patients were enrolled in this study.Serum HBV cccDNA and HBV DNA were determined by real-time fluorescence quantitative polymerase chain reaction（FQ-PCR）,while HBsAg and HBeAg were detected by chemiluminescence immunoassay.Results Among 83 cases,62 cases were HBV DNA positive.Among them,40 cases were HBV DNA load ≥105 copies/mL,28 cases（70.00%） were HBV cccDNA positive;22 cases were HBV DNA load 105 copies/mL,and 2 cases（9.09%） were HBV cccDNA positive.There was statistical significance in HBV cccDNA load between groups with different HBV DNA loads（P0.01）.The positive rates of HBV cccDNA in HBeAg positive and negative patients were 70.00%（28/40） and 4.65%（2/43） respectively.There was statistical difference（P0.01）.The quantitative value of HBV cccDNA was correlated positively with HBV DNA and HBsAg（P0.01）.Conclusions The serum level of HBV cccDNA is positively correlated with the levels of HBV DNA and HBsAg,and can reflect HBV replicative status and the efficacy of antiviral drugs. |
