纤维插管镜在麻醉及手术中的特殊应用

目的：探讨纤维窥镜在全麻插管及手术中的特殊应用价值。方法：分别在麻醉诱导、手术操作过程中以及气管导管拔出阶段，应用Pentax FI-10P2光导纤维插管镜，为12例病人进行受压气道的导管导向、定位、病灶观察、手术评估及协助隔离单肺通气。结果：3例病人用纤维插管镜引导气管导管通过了受压变形的气管段，术中通气管理满意。3例以纤维插管镜观察指引肿瘤切除手术范围，术后残端病理结果与术中判断一致。4例肺气肿病人进行了双腔气管导管定位并间断用纤维插管镜吸出肺内残余气体。结论：纤维插管镜在麻醉及手术中可用于导管导向、定位、病灶观察、手术评估及协助隔离单肺通气，术毕拔管时还可用于观察气管、支气管是否存在塌陷情况。

Special Application of Intubating Fibrescope During Anesthesia and Operation

Objective:To approach and widen the utilizing of fibrescope during anesthesia and oeprating period. Method:Pentan FI-10P2 intubating fibrescope was used in 12 patients for different puropse during anesthesia in duction,operation and extubation period.Results:Three trachea intubation were guided through narrowed trachea by intubating fibreacope.Operation field were viewed with intubating fibrescope to determine the remove range of tumor and that were verified after operation.Remain gas were draw out by intubating fibrescope in four wevere pulmonary emphysema patient.Conclusions:Intrubating fibrescope are very useful in anesthesia and operating period for verifying intubatin postition,observing operation field and drawing out the remain gas in lung of severe pulmonary emphysema patient.
