非手术治疗外伤性脾破裂30例临床分析

目的探讨非手术治疗脾破裂的适应证选择及临床效果。方法回顾性分析2002年5月至2008年5月非手术治疗30例脾破裂的临床资料。结果30例均治愈，非手术治疗成功27例，中转手术3例，治疗成功率90％。无脾脓肿，无延迟性脾破裂。24例随访6个月至5年无并发症。结论在严格的适应证范围内，非手术治疗外伤性脾破裂是安全、有效的治疗手段。

Non-operative Treatment of Splenic Rupture:Clinical Analysis of 30 patients

Objective To analyse the clinical efect and indication of non -operative treatment for splenic rupture. Methods From May 2002 to May 2008,30 cases with splenic rupture were treated by non - operative methods. The clinical data were reviewed retrospectively. Results 30 cases were all cured after examination of B - ultrasound,abdominal CT scan and abdominal puncture for clarifing diagnosis,in which 17 cases were of gradeⅠ injury,9 cases of grade Ⅱ ,3 cases of grade Ⅲ and 1 case of grade Ⅳ. 10 patients had associated injuries. 27 cases receiving conservative treatment, all cured（90 % ） ,3 patients were operated, no splenic abscess and delayed splenic rupture appeared. 24 cases were followed up for 6 months to 5 years,with no complication occurring. Conclusions Non - operative treatment of splenic rupture is safe and effective when indication was strictly controlled.
