血清铁蛋白在恶性血液病诊断与治疗中的价值

目的探讨血清铁蛋白（SF）检测在恶性血液病诊断与治疗中的价值。方法选择128例恶性血液病患者作为观察组,其中：急性髓性白血病（AML）35例（AML组）、急性淋巴细胞性白血病（ALL）21例（ALL组）、慢性髓性白血病（CML）28例（CML组）、慢性淋巴细胞性白血病（CLL）18例（CLL组）、骨髓增生异常综合征（MDS）26例（MDS组）;同时选择60例健康者作为对照组。检测对照组及观察组治疗前后SF水平。结果 AML组、ALL组、CML组、CLL组、MDS组治疗前及治疗完全缓解后SF水平均较对照组显著升高（P〈0.01）,完全缓解持续1年后SF水平与对照组相比,差异无统计学意义（P〉0.05）;AML组、ALL组、CML组、CLL组、MDS组治疗完全缓解及持续缓解1年后SF水平较治疗前均显著下降（P〈0.01）。结论恶性血液病患者体内存在明显的铁代谢紊乱,血清铁蛋白检测可作为白血病和MDS患者初治期、缓解期及预后的观察指标。

Value of Serum Ferritin in Diagnosis and Treatment of Malignant Hematopathy

Objective To investigate the value of serum ferritin（SF） detection in the diagnosis and treatment of malignant hematopathy. Methods A total of 128 patients with malignant hematopathy were divided into five groups： acute myeloid leukemia（AML） group（n=35）, acute lymphocytic leukemia（ALL）group（n=21）, chronic myelogenous leukemia（CML） group（n=28）, chronic lymphocytic leukemia（CLL）group（n=18）, and myelodysplastic syndrome（MDS） group（n=26）. In addition, 60 healthy subjects were selected as the control group. The levels of SF were measured before and after operation. Results Compared with healthy controls, SF levels significantly increased before treatment and after complete remission in patients with malignant hematopathy（P〈0.01）. No significant differences in SF levels were found between healthy controls and patients with malignant hematopathy after sustained remission for 1year（P〉0.05）. Compared with levels before treatment, SF significantly decreased after complete remission and sustained remission for 1 year in patients with malignant hematopathy（P〈0.01）. Conclusion Patients with malignant hematopathy have obvious disorder of iron metabolism. The SF level can be used as an indicator of initial treatment period, remission period and prognosis in patients with leukemia or MDS.
