幼年强直性脊柱炎骶髂关节的影像学检查

目的:探讨幼年强直性脊柱炎骶髂关节的CT和X线平片的表现。方法:收集57例14岁以下的强直性脊柱炎病例,对其双侧骶髂关节X线平片和CT片进行分析。结果:在57例114个骶髂关节中,无异常改变者31个关节,有异常改变者83个关节,表现为:骶髂关节间隙增宽,髂骨侧关节面毛糙、不规则,骨皮质不连续、变薄或增厚,关节面下囊状透光区,骨质硬化,骨质缺损。骶骨侧关节面改变较轻。结论:骶髂关节炎是幼年强直性脊柱炎的早期、主要的影像学改变。

Imaging Diagnosis of Sacroiliac Joint in Juvenile Ankylosing Spondylitis

Objective:To explore the CT and X-ray sign of sacroiliac joint in juvenile ankylosing spondylitis.Methods:We selected 57 cases with juvenile ankylosing spondylitis and their age is under 16,analyze their CT and X-ray sign detailedly of sacroiliac joint.Results In 114 sacroiliac joints of 57 cases with juvenile ankylosing spondylitis,there are 31 sacroiliac joints showed normal.There are 83 sacroiliac joints showed unusual.The CT and X-ray finding included:The sacroiliac joint space showed widened.The articular surface became indistinct,irregular,discontinued or thinned or thickened cortex.Subchondral cystic degeneration,bony destruction and sclerosis were seen.The above changes were less severe on sacral side than on iliac side.Conclusion:Sacroiliitis is an early stage and important manifestation in juvenile ankylosing spondylitis.
