微创闭合复位经皮空心加压螺丝钉及股骨近端抗旋髓内钉（PFN A）治疗股骨颈基底型骨折的临床疗效对比

目的：探讨微创闭合复位经皮空心加压螺丝钉及股骨近端抗旋髓内钉（PFNA）治疗股骨颈基底型骨折的临床疗效。方法选取我院自2009年9月至2014年12月期间收治的股骨颈基底型骨折60例，按照手术治疗方法分为空心加压螺丝钉组36例和PFNA组24例。对这两种不同术式所显示的手术时间、术中出血量、术后下床时间、术后骨折愈合时间、术后并发症发生率及术后患者髋关节功能（Harris评分）等方面的指标进行对比分析。结果与空心加压螺丝钉组相比，PFNA组手术时间长、术中出血量多、术后卧床时间短、骨折愈合时间短，术后并发症发生率低（P＜0.05）。两组患者术后髋关节功能评分其中PFNA组较空心加压螺丝钉组相比术后髋关节功能评分明显高（P＜0.05）。结论采用PFNA治疗股骨颈基底型骨折的临床疗效显著，术后髋关节功能恢复情况较好，安全性高，值得推广。

A comparative study of treatment efficacy between cannulated compression screws after closed reduction and PFNA

Objective To investigate the clinical effect of the treatment of femoral neck basal-type fracture by using the ma-nipulative reduction with percutaneous hollow compression screw and PFNA .Methods Sixty cases with femoral neck basal-type fracture in our hospital form September 2009 to December 2014 were retrospectively reviewed .According to the surgical treatment ,it can be divided into two groups :hollow compression screw group(n=36) and PFNA group(n=24) .This thesis compares these two methods from the following indexes:time of each operation ,intraoperative bleeding ,postoperative activity time ,time of fracture union ,post-operative complications probability and function of postoperative hip that is judged based on Harris Hip Joint Function Scale .Results Comparing with the hollow compression group ,the time of operation in PFNA group is longer ,interoperative bleeding is more ,bedridden time after operation is shorter and time of fracture union is also shorter ,and the rate of post-operative complications probability is lower(P<0.05) .Post-operative fineness rate in PFNA group is obvious-ly higher than that in hollow compression group(P<0.05) .Conclusion The clinical treatment of femoral neck basal-type frac-ture by using PFNA has striking effect and the recovery of hip joint function after surgery is very well .And it has high security and is worth spreading .
