腋下小切口在自发性气胸的临床应用体会

目的探讨经腋下小切口开胸治疗自发性气胸的临床应用价值.方法对2005年2月至2009年12月采用腋下小切口治疗的23例肺大疱合并自发性气胸的临床资料进行回顾性分析.术中对较大的肺大疱采用楔形切除、重叠褥式缝合;较小的肺大疱予以结扎.结果 23例术后1～3d拔除胸腔引流管,均治愈,随访3～24个月无气胸复发病例.结论经腋下小切口开胸手术治疗自发性气胸安全,创伤小、恢复快,疗效可靠.

Clinical Application of the Axillary Thoracotomy in the Treatment of Spontaneous Pneumothorax

Objective To explore the clinical value of the axillary thoracotomy incision in patients with spontaneous pneumothorax.Methods A retrospective analysis was conducted of the clinical data of the 23 cases of patients with spontaneous pneumothorax caused by ruptured pulmonary bullae who were treated with axillary thoracotomy from Feb 2005 to Dec 2009.The larger bullae were treated with the methods of wedging and overlapping mattress sutures,while the smaller bullae were ligated.Results The intrathoracic drains of all the 23 patients were removed 1 to 3 days after the surgeries and all the patients were cured.No recurrent pneumothorax was found in the follow-ups 3 to 24 months after the surgeries.Conclusion Axillary thoracotomy incision has the characteristics of reliable safety,small trauma,quick recovery and reliable efficacy in the treatment of spontaneous pneumothorax.
