腹腔镜技术在急性化脓性腹膜炎诊断和治疗中的作用

目的探讨腹腔镜技术在急性化脓性腹膜炎中诊断和治疗的作用.方法对104例化脓性腹膜炎患者实施腹腔镜探查,对腹腔镜手术实施率、诊断正确率、治愈率、中转手术率进行回顾性分析.结果腹腔镜手术实施率95.2%,腹腔镜术中诊断正确率为98.1%,诊断与治疗可经腹腔镜一次操作完成,治愈率97.9%,中转开腹率4.8%.结论腹腔镜技术运用于急性化脓性腹膜炎诊断和治疗是可行、安全、有效的,具有诊断正确率高、损伤小、术中探查范围广等优点;腹腔镜手术在急腹症中目前尚不能完全取代开腹探查手术.

Application of laparoscopy in the diagnosis and treatment of acute suppurative peritonitis

Objective To evaluate the application of laparoscopic techniques in the diagnosis and treatment of acute suppurative peritonitis.Method 104 cases of acute suppurative peritonitis were treated with laparoscopy. The clinical data were reviewed retrospectively, including the rate of application, the rate of accurate diagnosis and the healing rate.Results The rate of application with laparoscopy was 95.2%, the rate of accurate diagnosis was 98.1% and the healing rate was 97.9%. The rate of open operation was 4.8%. The diagnosis and treatment were completed at one time with laparoscopy. Conclusion Laparoscopy may be used in the diagnosis and treatment of acute suppurative peritonitis. This technique is safe and has many advantages such as high rate of accurate diagnosis, less injuriy, clear inspection of the intra-abdominal organs. But open operation is not able to be taken place by laparoscopic operation completely now. Laparoscopic operation need to be performed by experienced general surgeons.
