Breath-link呼吸功能评估在老年卒中病人吸气肌功能评价中的应用研究

摘    要:目的研究Breath-link呼吸功能评估在老年卒中病人吸气肌功能评价中的应用价值。方法选取30例老年卒中病人,按简易智力状态检查量表(MMSE)评分将病人分为认知功能正常组(16例)和轻度认知障碍组(14例),2组病人在常规康复训练的同时行呼吸功能训练,收集并比较2组受试者治疗前后不同呼吸时相(平静呼吸、深吸气)膈肌厚度、移动度及吸气肌功能(吸气峰流速),同时对比Breath-link呼吸功能评估与膈肌超声对膈肌功能障碍诊断的灵敏度、特异度和准确度。结果治疗后2组吸气峰流速、膈肌增厚率和膈肌位移较治疗前均明显改善,差异具有统计学意义(P<0. 05);认知功能正常组改善更明显,2组比较差异具有统计学意义(P<0. 05)。Breath-link呼吸功能评估对膈肌功能障碍诊断的灵敏度、特异度和准确度明显低于膈肌超声,差异有统计学意义(P<0. 01)。结论 Breath-link呼吸功能评估对膈肌功能障碍诊断的灵敏度、准确度明显低于膈肌超声,认知障碍病人膈肌功能改善较差。

Abstract:Objective To study the value of Breath-link respiratory function assessment on inspiratory muscle function in the elderly patients with stroke.Methods Thirty elderly patients after stroke in our hospital were selected and divided into normal cognitive function group (16 cases) and mild cognitive impairment group (14 cases) according to Mini-mental State Examination (MMSE) score. Respiratory function training and routine rehabilitation training were performed in two groups. The diaphragm thickness and mobility, inspiratory muscle function (inhalation peak flow rate) during quiet breathing and deep breathing before and after treatment were detected.Meanwhile,the sensitivity, specificity, and accuracy in the diagnosis of diaphragmatic dysfunction by Breath-link respiratory function assessment and diaphragm ultrasound were compared.Results After treatment, the inspiratory peak flow rate,diaphragm thickness ratio and diaphragm displacement in two groups were significantly improved ( P < 0.05),especial in normal cognitive function group ( P < 0.05).The sensitivity and accuracy of Breath-link respiratory function assessment in the diagnosis of diaphragmatic dysfunction were significantly lower than those of diaphragmatic ultrasound ( P < 0.01).Conclusions The sensitivity and accuracy of Breath-link respiratory function assessment in the diagnosis of diaphragmatic dysfunction are lower than those of diaphragmatic ultrasound,and the improvement of diaphragmatic function in the patients with cognitive impairment is poor.
