体检人群幽门螺杆菌感染者血清葡萄糖、脂类及同型半胱氨酸水平与Hp抗体分型的相关分析

摘    要:目的 分析深圳地区体检人群幽门螺杆菌(Hp)感染及分型与血清葡萄糖、脂类及同型半胱氨酸异常水平的相关性。方法 选取2017年10~12月南方医科大学深圳医院健康体检者共2 256例作为研究对象,采用免疫印迹法检测Hp抗体,全自动生化分析仪检测血糖(GLU)、血清总胆固醇(TC)、三酰甘油(TG)、高密度脂蛋白(HDL)、低密度脂蛋白(LDL)及同型半胱氨酸(HCY)。依据Hp抗体结果分析体检人群Hp感染的性别、年龄分布特点,同时把研究对象分为Hp抗体阴性组、II型Hp抗体阳性组、I型Hp抗体阳性组三组,并对这三组体检人群的血清GLU,TC,TG,HDL,LDL及HCY水平进行比较分析。结果 ①体检人群Hp感染率男性(55.9%,618/1 106)与女性(44.1%,488/1 106)比较,差异有统计学意义(χ2=31.10,P<0.05); 阳性组平均年龄47.04±12.68岁与阴性组43.86±11.05岁比较,差异有统计学意义(t=1.97,P=0.01<0.05)。②体检者Hp I型抗体阳性组与Hp抗体阴性组比,TG水平差异有统计学意义(F=6.22,P<0.05),GLU,TC,HDL,LDL及HCY水平差异均无统计学意义(F=0.040.44,P>0.05); Hp II型抗体阳性组和Hp抗体阴性组以及Hp II型抗体阳性组和Hp I型抗体阳性组两两比较,GLU,TC,TG,HDL,LDL及HCY水平差异均无统计学意义(F=0.012.65,P>0.05)。③体检者Hp I型抗体阳性组与Hp抗体阴性组比较,GLU,TC,TG和HDL异常率差异均有统计学意义(χ2=4.7438.60,P<0.05),LDL和HCY异常率差异均无统计学意义(χ2=0.170.30,P>0.05)。Hp II型抗体阳性组与Hp抗体阴性组比较,TG和HDL异常率差异均有统计学意义(χ2=5.0716.43,P<0.05),GLU,TC,LDL及HCY异常率差异均无统计学意义(χ2=0.021.26,P>0.05)。Hp I型抗体阳性组和Hp II型抗体阳性组各项目两两比较,GLU,TC,TG,HDL,LDL及HCY异常率差异均无统计学意义(χ2=0.012.89,P>0.05)。结论 深圳地区体检者Hp感染男性高于女性,I型Hp感染与GLU,TC,TG和HDL异常有一定的相关性,II型Hp感染与TG和HDL异常相关,不同型Hp感染对血糖血脂的影响不同,Hp感染进行分型检测有重要的临床意义。

Abstract:Objective To analyze the relationship betweenHelicobacter pylori(Hp)infection and different types of serum glucose,lipid and homocysteine abnormalities in the physical examination population in Shenzhen area.Methods A total of 2 256 healthy subjects wereselected from October 2017 to December 2017 in Shenzhen Hospital of Southern Medical University, with the immunoimprinting method to detect Hp antibodies,automatic biochemical analyzer for blood glucose(GLU),serum total cholesterol(TC), triacylglycerol(TG),high density lipoprotein(HDL),low density lipoprotein(LDL)and homocysteine(HCY).According to the results of Hp antibody,analysed the gender and age distribution characteristics of Hp infection in the physical examination population and divided into three groups:Hp antibody negative group,type II Hp antibody positive group and type I Hp antibody positive group,the serum GLU,TC,TG,HDL,LDL and HCY levels of these three groups were compared and analyzed.Results ①Hp infection rate in male patients(55.9%,618/1 106)compared with female(44.1%,488/1 106),the differencewas statistically significant(χ2=31.10,P<0.05),and the average ageof the positive group(47.04±12.68 age)compared with the negative group(43.86±11.05 age),the difference was statistically significant(t=1.97,P=0.01<0.05).②The positive group of Hp I antibody was compared with the negative group of Hp antibody,the difference in TG level was statistically significant(F=6.22,P<0.05),and the differences in GLU,TC,HDL,LDL and HCYlevels were all no statistically significant(F=0.040.44,all P>0.05).Between the Hp II antibody positive group and the Hp antibody negative group,and the Hp type II antibody positive group and the Hp type I antibody positive group,the differences in GLU,TC,TG,HDL,LDL and HCY were all no statistically significant(F=0.012.65, all P>0.05).③The positive groupof Hp I antibody was compared with the negative group of Hp antibody,the abnormal rates of GLU,TC,TG,and HDL were statistically significant(χ2=4.7438.60, all P<0.05),LDL and HCY abnormalities were not statistically significant(χ2=0.170.30, all P>0.05).The Hp II positive groupcompared with Hp negative group,the abnormal rates of TG and HDL were all statistically significant(χ2=5.0716.43, all P<0.05),and the abnormal rates of GLU,TC,LDL and HCY were all no statistically significant betweenthe two groups(χ2=0.021.26, all P>0.05).Pairwise comparisonof eachitem between the Hp I antibody positive group and the Hp II antibody positive group,the abnormal rates of GLU,TC,TG,HDL,LDL and HCY were all no statistically significant(χ2=0.012.89, all P>0.05).Conclusion The Hp infection in males in Shenzhen area was higher thanthat in females.There was a certain correlation between type I Hp infection and abnormalities of GLU,TC,TG and HDL.Type II Hp infection was associated withabnormalities of TG and HDL.Different types of Hp infection had different effects on blood glucose and blood lipids.It is of great clinical significance to classify and detect Hp infection.
