丹参多酚酸盐注射液辅助介入治疗冠心病及对患者心、肾功能和心肌损伤的影响

摘    要:目的:探讨丹参多酚酸盐注射液辅助经皮冠脉介入治疗(PCI)对冠心病患者心功能、肾功能及心肌损伤的影响,为冠心病临床诊治提供参考。方法:选取需行PCI手术治疗的冠心病患者80例,将患者随机分为对照组与研究组两组,每组40例,两组患者均行PCI手术治疗,术前及术后常规用药,研究组在围术期同时给予丹参多酚酸盐注射液静脉滴注治疗,观察患者术后1个月内主要心血管事件(MACE)发生情况,患者术前与术后7 d时左室射血分数(LVEF)、左室舒张末期容量(LVEDV)、左室收缩末期容量(LVEsV)、血清肌酐(Scr)、胱抑素c(Cys-C)变化,术前及PCI术后24 h血肌钙蛋白I(cTnI)、肌酸激酶同工酶(CKMB)、脑钠肽(BNP)变化。结果:对照组术后发生8例MACE,MACE发生率为20.00%,研究组术后发生2例MACE,MACE发生率为5.00%,差异有统计学意义(P<0.05);两组术前LVEF、LVEDV、LVEsV比较差异无统计学意义(P>0.05),术后7 d时两组LVEF均较术前明显升高(P<0.05),且研究组高于对照组(P<0.05),LVEDV、LVEsV均较术前下降(P<0.05),且研究组低于对照组(P<0.05);两组术前血Scr、Cys-C比较差异无统计学意义(P>0.05),术后7 d时两组Scr、Cys-C均较术前明显升高(P<0.05),且对照组高于研究组(P<0.05);两组术前cTnI、CKMB、BNP比较差异无统计学意义(P>0.05),术后24 h时两组cTnI、CKMB、BNP均较术前明显升高(P<0.05),且对照组高于研究组(P<0.05)。结论:PCI治疗冠心病在围术期给与丹参多酚酸盐注射液辅助治疗,有助于改善患者术后心功能、肾功能,减轻心肌损伤。

Abstract:Objective:To investigate the effect of salvianolate injection on cardiac function,renal function,myocardial injury in percutaneous coronary intervention(PCI)postoperative patients with coronary heart disease,and provide the basis for clinic treatment of patients with coronary heart disease.Methods:We selected 80 cases of patients need PCI with coronary heart disease,they were randomly divided into control group(40 cases)and observation group(40 cases),and the two groups received PCI treatment,the observation group extra salvianolate injection for treatment in perioperative period.To observed major cardiovascular events(MACE)occurred in within 1 month after surgery,left ventricular ejection fraction(LVEF),left ventricular end-diastolic volume(LVEDV),left ventricular end-systolic volume(LVEsV),serum creatinine(Scr),cystatin c(Cys-c)in before and 7 d after surgery,serum troponin I(cTnI),creatine kinase isoenzyme(CKMB),brain natriuretic peptide(BNP)in before and 24h after surgery.Results:8 cases of MACE occurred postoperatively in the control group(20.00%),2 cases of MACE occurred postoperatively in the control group(5.00%),the difference was statistically significant(P<0.05).LVEF,LVEDV and LVEsV before surgery were not statistically significant between the two groups(P>0.05),LVEF of after surgery was significantly higher than those of before surgery of patients with the two groups(P<0.05),and the observation group was higher than the control group(P<0.05),LVEDV and LVEsV of after surgery were significantly lower than those of before surgery of patients with the two groups(P<0.05),and the observation group were lower than the control group(P<0.05).Scr and Cys-C before surgery were not statistically significant between the two groups(P>0.05),Scr and Cys-C of after surgery were significantly higher than those of before surgery of patients with the two groups(P<0.05),and the observation group was higher than the control group(P<0.05).cTnI,CKMB and BNP before surgery were not statistically significant between the two groups(P>0.05),cTnI,CKMB and BNP of after surgery were significantly lower than those of before surgery of patients with the two groups(P<0.05),and the observation group was lower than the control group(P<0.05).Conclusion:PCI for the treatment of coronary heart disease was administered with salvianolate injection in the perioperative period,it is helpful to improve the cardiac and renal function and reduce the myocardial injury.
